〔研究目的〕母乳移行率の指標であるMilk/Plasma (M/P)およびRelative Infant Dose (RID)を算出することを目的とし、ロフラゼプ酸エチルの活性代謝物の血中濃度及び母乳中濃度測定系の構築すると共に、臨床試験プロトコルの立案を行った。〔研究成果〕1. 測定条件の確立ロフラゼプ酸エチル活性代謝物を含むヒト血漿もしくは粉ミルクに1N HC1を加え混和し、30分間室温にて静置後、10% NH_4OHを加え混和した。さらに、内部標準溶液CM7113を添加し、攪拌後、遠心分離した。得られた上清について固相抽出処理を行い、LC-MS/MS法にて薬物濃度を測定した。ロフラゼプ酸エチルの活性代謝物濃度0.5-500ng/mLの範囲において良好な直線性が得られた。血漿中活性代謝物の場合はR^2=0.9741、乳汁中の場合はR^2=0.9976となり、精度よく測定できる測定系を確立した。2. プロトコルの立案日常臨床においてロフラゼプ酸エチルを使用している授乳中の患者とその児を対象とし、文書による説明と同意の取得を行う。母体血液および母乳の採取は服薬直前に、乳児採血は新生児マススクリーニング用採血に合わせて実施し、濃度測定を行う。さらに、母体の催眠効果、健忘やせん妄などの有害事象、乳児の回帰等の情報を収集し、血中濃度との関連について検討するプロトコルを立案した。3. 倫理審査の承認京都大学大学院医学研究科・医学部および医学部附属病院医の倫理委員会に本研究計画を提出し、承認された(承認番号 : R0402)。〔今後〕今後、臨床検体を用いた測定に移行する。

[Objective] To establish a system for measuring the serum concentration of active metabolites of breast milk and breast milk concentration, and to establish a clinical trial system for calculating the milk migration rate index, Milk/Plasma (M/P) and Relative Infant Dose (RID). The patient's daily clinical use of milk is related to the document and consent is obtained. Maternal blood and breast milk are collected in response to treatment, neonatal blood collection, neonatal blood collection, and concentration determination. Information on maternal hypnotic effects, amnesia, adverse events, and breast return is collected, and the correlation between blood concentration and risk is discussed. 3. Ethics Review Approval This research plan was submitted and approved by the Ethics Committee of Kyoto University Graduate School of Medicine, Faculty of Medicine and Affiliated Hospital of Faculty of Medicine (Approval No.: R0402). In the future, clinical trials will be conducted.
