项目摘要

【研究目的】ワルファリンは、ビタミンKと拮抗することで抗凝固作用を示す薬剤であり、相互作用する薬剤が多く、プロトロンビン時間国際標準比(PT-INR値)を用いた維持投与量の調節が推奨されている。脂肪乳剤は、効率の良いエネルギー補給の目的でしばしば臨床使用されているが、近年、脂肪乳剤とワルファリンの併用は、ワルファリンの抗凝固能に影響を与えるという症例が報告されている。そこで、ワルファリン抗凝固能に対する脂肪乳剤の影響についての詳細を検討するため、カルテ情報に基づいた後方視的調査を行った。【研究方法】2011年10月1日から2014年3月31日の2.5年間に京都大学医学部附属病院にてワルファリンと脂肪乳剤を併用した症例をカルテから抽出した。なお、本調査は本院医の倫理委員会の承認を得て行った。【研究成果】解析対象となった7症例のうち、脂肪乳剤との併用によりPT-INR値が減少傾向を示した症例は3症例であった。この3症例のPT-INR値について詳細に解析すると、ワルファリン単独投与時は1.82-2.78であり、脂肪乳剤併用時は1.18-1.29であった。さらに、併用前後でワルファリン投与量が変更されている例もあるため、PT-INR値をワルファリン投与量で補正したWarfarin Sensitivity Index(WSI)値で評価した。その結果、WSI値は脂肪乳剤を併用することで減少傾向を示した。一方、併用の影響を示さなかった4症例においては、WSI値についても併用前後の値で変動は認められなかった。以上、ワルファリンの抗凝固能は、脂肪乳剤との併用により減弱する可能性が示唆された。脂肪乳剤の併用による影響を受けない症例も見られるものの、ワルファリン使用時には、脂肪乳剤に含まれるビタミンKの影響も考慮する必要があると考える。

[Objective] To maintain the regulation of dosage by using the PT-INR value, which is the international standard ratio (PT-INR) of anti-coagulation effect, anti-coagulation effect and anti-coagulation effect. The purpose of fat emulsion is to improve the quality of the product. In recent years, fat emulsion has been used in combination with anti-coagulation effect. A detailed study of the effects of anti-coagulation properties on fat emulsion was conducted. [Method] Cases of combined use of fat emulsion and fat emulsion were reported in Kyoto University Hospital during the 2.5 years from October 1, 2011 to March 31, 2014. This investigation was approved by the Ethics Committee of our hospital. [Results] Analysis of 7 cases, fat emulsion and combination of PT-INR decreased tendency, 3 cases The PT-INR value of these three cases was analyzed in detail. The time of single injection was 1.82-2.78, and the time of combination of fat emulsion was 1.18-1.29. Before and after the war, the amount of investment was changed. For example, the PT-INR value was changed. The Warfarin Sensitivity Index(WSI) was revised. The results showed that WSI had a tendency to decrease in fat emulsion use. The influence of one party and combination is shown in 4 cases, and WSI value is shown in 4 cases. The above results indicate that the anti-coagulation properties of fat emulsion and fat emulsion may be reduced.
