研究目的 :高齢者にとって運動機能の低下は直接日常生活レベルの低下へとつながる恐れがあり、特に体幹の柔軟性を維持することは転倒や障害を防ぐ上では非常に重要と考えられる。鍼灸治療における鍼刺激は、四肢の経穴(ツボ)への簡単な刺激で身体の柔軟性を高める可能性があることが知られているが、刺激方法による効果の違いは明確にされていない。今回の研究では、通常行われる鍼刺激と皮膚及び皮下組織のみを刺激する円皮鍼(鍼長0.3mmの貼付型の鍼)を用いて、上肢・下肢の代表的な経穴への刺激が体幹柔軟性に与える影響を比較した。研究方法 :健常成人6名(平均年齢35.3歳)を対象に、①コントロール(無刺激)、②-a. 上肢の経穴「合谷」への通常の鍼刺激を10秒間、②-b. 下肢の経穴「足三里」への通常の鍼刺激を10秒間、③-a. 「合谷」へ円皮鍼を皮膚に貼付、③-b. 「足三里」へ円皮鍼を皮膚に貼付、の3パターンの方法を行い、刺激前後の体幹柔軟性の変化を立位体前屈での指床間距離(FFD)で評価した。各方法はそれぞれ別の日に行い、刺激前後にはストレッチ効果を除外するため1時間以上の間隔を空けた。研究成果 :コントロールでは、FFDの変化は平均-2mmとほとんど変化しなかったのに対し、通常の鍼刺激と円皮鍼の貼付では、上肢「合谷」においても下肢「足三里」においても共にFFDが平均0.9～23mm変化し、体幹の柔軟性が高まる傾向が認められた。特に上肢「合谷」の通常の鍼刺激は平均23mmと統計学的に有意なFFDの変化が認められた。今回の結果から鍼刺激は一様に体幹の柔軟性を高める効果が期待出来ると推察され、皮膚及び皮下組織のみへの刺激より通常の鍼刺激がより効果的である可能性が示唆された。しかし、被験者に若年者が多いことと、変化の有無には個体差が大きいことが検討課題と思われた。

Objective: It is very important to maintain the flexibility of the trunk of the body and prevent the damage of the body. In this study, we compared the effects of needle stimulation, skin and subcutaneous tissue stimulation, skin needle (needle length 0.3 mm), nerve point stimulation, and body trunk softness on the skin, upper limbs and lower limbs. Methods: 6 healthy adults (average age 35.3 years) were selected as subjects. The upper limb nerve point "Hegu" and the usual needle stimulation for 10 seconds, ②-b. The acupuncture point of lower limb "Zusanli" is stimulated by acupuncture for 10 seconds, ③-a. "Hegu" skin needle skin paste, ③-b. "Zusanli" skin needle skin attachment, three different methods, the change of body trunk flexibility before and after stimulation, the inter-finger distance (FFD) of standing body flexion were evaluated. Each method is different from the previous day, except for the time interval before and after stimulation. The results of the study are as follows: the average diameter of FFD is-2mm, the average diameter of FFD is-2mm. Special upper limb "joint valley" of the usual needle stimulation is an average of 23mm, statistical analysis of the intention of FFD and recognition. The results of this study show that the stimulation of the needle is expected to increase the softness of the body shaft, and the stimulation of the skin and subcutaneous tissue is likely to increase the effectiveness of the needle stimulation.
