○研究目的:我々は肩凝り有訴者の多くは軽度の可動域制限がみられること、上肢の感覚過敏や鈍磨などが起きている可能性があることを推測している。今回の研究では、これまでとは異なる視点から肩凝りを把握することと、肩凝りに対する物理療法の効果を検討することを目的とした。○研究方法:1、肩凝りの部位、症状を調ベタイプ別に分類する。2、全身広範囲な関節のアライメントや可動性を調べるために、頚椎・肩関節・肩甲骨・腰椎・股関節の可動域を測定し、症状との関連を見出す。3、上肢の感覚障害の有無を調べるため、電流知覚閾値装置により知覚閾値を測定する。4、物理療法として、温熱療法(ホットパック)及び鍼治療を行い、刺激前後の知覚閾値の変化を調べる。○研究成果:1、肩凝りの部位は肩上部、肩甲間部、頚部の順に多かった。2、肩凝りの強い10名を対象に、各部の関節可動域を測定した結果、頚椎では肩凝りを自覚している側(以下患側)とは反対側(以下健側)への動きが制限されている者が多かった(7名)。肩関節では患側可動域が健側より大きい者が多く(8名)、逆に肩甲骨の前傾は患側に強い傾向がみられた(6名)。このことは、肩甲帯の緊張により肩甲骨の可動性は制限されるものの、代償的に上腕の可動性が高まっているものと推測された。腰部、股関節の可動域については肩凝りとの明らかな関連を見出せなかった。3、上記の中から同意の得られた者7名を対象に、第3指の電流知覚閾値を測定した結果、患側の閾値が高かった者5名、患側の閾値が低かったもの2名と、患側の知覚が鈍くなっている者が多い傾向がみられた。4、更に治療の同意を得られた5名に対して、患部へ10分間ずつの温熱療法、鍼治療を行った。温熱療法では2名に知覚閾値の変化がみられたのに対し、鍼治療では5名全例に変化が見られ、特に触圧覚を伝えるAβ線維が最も変化しやすい傾向であった。

Physical therapy, thermotherapy and acupuncture therapy, and the change of cognitive threshold before and after stimulation.○ Research results:1. The shoulder joint has a strong tendency to tilt forward and backward (6 cases). More treatment consent was obtained from 5 patients, and the affected part was treated with thermotherapy and acupuncture for 10 minutes. Thermotherapy has two main characteristics, namely, the change of threshold value, the change of acupuncture treatment, and the change of Aβ line dimension.
