项目摘要

○研究目的 : 近年、眼科領域でのサイトメガロウイルス感染症治療に、院内製剤の0.5%ガンシクロビル点眼液が汎用されている。長崎大学病院(以下、当院)では、これまでに0.5%ガンシクロビル点眼液の安定性や安全性に関する調査を行い報告している。今回、当院で高濃度の2.0%ガンシクロビル点眼液が使用開始となったことから、2.0%ガンシクロビル点眼液の使用患者での副作用の発現状況について調査した。また、院内製剤としての安全性評価のために、in vitroにおいて角膜への障害性を培養細胞で評価した。○研究方法 : 当院において2015年4月から12月の間で2.0%ガンシクロビル点眼液が処方された患者12名を対象とし、使用感および眼圧上昇の有無をカルテから後方視的に調査した。一方、in vitro試験においては、0.5%および2.0%のガンシクロビル点眼液をウサギ角膜細胞に5分間曝露させ、細胞生存率を測定した。角膜障害性の評価は、短時間曝露試験法に準じ、細胞生存率が70%以下のものを障害有りと判定した。○研究成果 : 2.0%ガンシクロビル点眼液の副作用の発現については、3名の患者で点眼初期に軽い刺激感が認められたのみで、使用中止となるような重篤な副作用は発生しなかった。また、ガンシクロビル点眼液は0.5%、2.0%いずれの濃度においても角膜細胞への障害性は認められなかった。本結果より、2.0%ガンシクロビル点眼液は臨床使用において、安全性の高い製剤であることが示唆された。本研究成果は、患者個人の治療の有用性のみならず、ガンシクロビル点眼液の今後の臨床使用における安全性の構築にも貢献できる重要な研究である。なお、本研究成果は、医療薬学フォーラム2016(滋賀)にて既にポスター発表を行っている。現在は、本研究成果を薬学雑誌へ投稿し、査読中の状態にある。

○ Research purpose: In recent years, 0.5% Gansikrobilu eye drops prepared in the hospital have been widely used in the field of ophthalmology for the treatment of infectious diseases. Nagasaki University Hospital (hereinafter referred to as "Nagasaki University Hospital") conducted a study on the stability and safety of 0.5% Nagasaki Eye Drops. Investigation on the occurrence of side effects in patients using high concentration of 2.0% In addition, the safety evaluation of the in-hospital system and the evaluation of the damage of corneal cells in vitro○ Method: 12 patients with eye drops prescribed by our hospital from April to December 2015 were investigated for their sensitivity to use, elevated ocular pressure, and presence or absence of eye drops. One side, in vitro test, 0.5% and 2.0% of the eye drops were used to expose corneal cells for 5 minutes, and the cell survival rate was determined. Corneal impairment assessment, short-term exposure test method is accurate, cell survival rate is less than 70% and there is no impairment determination.○ Research results: 2.0% of eye drops had side effects, 3 patients had irritation at the initial stage of eye drops, and the use was discontinued. The concentration of the eye drops was 0.5% and 2.0% respectively. The results show that 2.0% of the eye drops are safe for clinical use. The results of this study are important research contributions to the efficacy of individual patient therapy and to the safety of future clinical use of Ophthalmic Solution. The results of this study are expected to be published in 2016(Shiga). At present, the results of this research are submitted to the academic journal, and the status of research is discussed.
