【目的】検査過誤防止のためにABO,Rho(D)血液型検査には、カラム凝集法を用いた自動化法が導入されている.ABO,Rho(D)血液型以外の血液型抗原検査も骨髄移植の生着判定や不規則抗体保有患者の血液型抗原を検査することがある.しかし抗血清は高価で,検査する患者も輸血を実施している場合が多く,判定が困難な場合がある.そこで,希釈した市販の試験管法検査用抗血清(抗C,c,E,e,M,N,Pl,Le^a,Le^b,Fy^a,Fy^b,Jk^a,Jk^b,S,s血清)25μlと被検血球50μlをゲルカラムカード上で最も良好な条件を設定して,安価で判定が容易な方法の開発を行なった.【方法】1%アルブミン生理食塩水で100倍希釈した抗C,c,E,e,血清は,オリンパス社NaCl/Enzymeカードで37℃15分,1%アルブミン生理食塩水で10倍希釈した抗Fy^a,Fy^b,Jk^a,Jk^b,S,s血清は,IgGカードで37℃15分,また1%アルブミン生理食塩水で10倍希釈レた抗M,N,Pl,Le^a,Le^b血清は,抗Pl,Le^a,Le^b血清には反応増強剤としてブロメリンを添加してNaCl/Enzymeカードで室温15分反応後,900g,10分の遠心判定した.【結果】希釈した抗血清は,抗Jk^aおよび抗e血清で,わずかな劣化がみられたが,他の抗血清は6ケ月間は安定であった.また骨髄移植前後に血液型抗原検査依頼のあった患者検体10本について,試験管法とゲルカード法の結果の一致率は,Rh血液型100%,MNSs血液型98%,kidd血液型100%,Duffy血液型85%,Le血液型93%,Pl血液型100%と良好であった.また輸血を実施した患者の血液型は,試験管法では輸血による影響で判定が困難であるが,ゲル・カラム法ではダブル・ポピュレーションを示すので判定が容易であった.【考察】市販の抗血清の使用量を少なくしてABO型以外の血液型抗原検査をゲル・カラム法で検査することで,検査費用も約3分の1程度で可能であった.

[Results] Antiserum Jk^a e According to the results of 10 samples of patients before and after bone transplantation, the concordance rate of blood type antigen test by tube test was 100% for Rh blood type, 98% for MNSs blood type, 100% for kidd blood type, 85% for Duffy blood type, 93% for Le blood type, and 100% for Pl blood type. It is difficult to determine the blood type of the patient who received the blood transfusion by the blood test tube method, but it is easy to determine the blood type by the blood test tube method. [Investigation] The amount of antiserum used in the market is relatively small, and the detection cost of blood type antigen test other than ABO type is about 3 minutes and 1 degree.
