项目摘要

日本人の0.5%はRhD抗原が陰性であるが, 吸着解離試験を実施することで抗原性が確認できるDel型が存在する. 吸着解離試験は, 4倍希釈抗D血清を吸着後, ジクロロメタン・ジクロロプロパン解離液で解離する改良を実施した. 一方, RhD血液型の高速DNAタイピングは, 日本赤十字社より情報提供を受けたシステムを一部改変しRHD欠損と日本人で頻度の高いDel変異アリル(1227G>A, K409K)を同時に増幅可能なmultiplex-PCRを用いた. 血清学的検査でRhD陰性と判定した170例のうち140例に吸着解離試験を実施して14症例のDel型を検出した. 全例がC抗原陽性で, 27症例のC抗原陽性の約52%であった. さらに, 血清学的検査でRhD陰性と判定した59症例を対象に高速DNAタイピングを実施した. このうち8例(13%)が血清学的にDel型と判定され, multiplex-PCRによる高速DNAタイピングの結果は, 全てで1227G>A変異のアリル特異的PCRのバンドが陽性であり, 1227G>Aの遺伝子型であると判定された. 血清学的にRhD陰性でありながらPCRでRHD陽性バンドを認めたケースはsingleplex-PCRによりRHDの全エクソンを確認した. 血清学的にRhD陰性型51例中、49例はRHD欠損型、2例はRHD-CE-DハイブリッドであるRHD (1-2)-CE (3-9)-D (10)であった. 臨床的には血清学的にRhD陰性と判定された患者にDel型血液を輸血した場合の不規則抗体産生のリスク, および輸血が実施されたDel型の患者に抗体産生が起きていないかを明確にする必要があった. 今回, Del型と判定された14患者のうち1患者でRh陰性赤血球4本, Rh陰性血小板2本, およびRh陽性血小板2本が輸血されたが抗体の産生は確認されなかった.

Among 51 serological RhD negative cases, 49 cases were RHD deficient, 2 cases were RHD CE D deficient, 2 cases were RHD (1-2) CE (3-9) D (10) negative.
