项目摘要

知的障害を主とする特別支援学校の保健室来室記録から、特別支援学校における健康課題を分析した。課題解決方法を「痛みや不安の軽減、関心・意欲を引き出す、自ら決定し実行する、人とかかわりながら育つ、学習内容の循環、組織的な活動、子育て支援」に分類し、4つの健康教育プログラムを開発した。児童生徒・保護者・担任教諭・養護教諭・学校医・医療機関関係者が協働してプログラムを実践し健康課題解決を試みた。実践後、児童生徒の変容、評価表、関係者からの聞き取り、記述式調査から成果を省察し、校内研究組織・横浜国立大学教育人間科学部附属学校養護教諭部会・学会で発表し検討した。「安心と自信を育む健康診断学習プログラム」では、視覚的教材と実物を使用した事前指導、検診時の養護教諭や学校医の継続的なかかわりが不安や恐怖の軽減を援助した。できたことへの賞賛の言葉や花丸確認カードが児童生徒のプラスの経験を印象づけ、次への意欲につながった。「自分も他の人も大切にする感染症対策プログラム」では「かからない、広げない」をスローガンに、家庭・学校・地域が組織的に実践した。保健便りやHPでの情報提供、朝・昼・夕の体温測定、児童生徒会主催の健康集会活動、集団への保健学習、個別の保健指導を循環させることで、新型インフルエンザの蔓延重症化防止の成果を得た。「歯科保健学習を通して学ぶ医療系学生と障害者双方向の医療リテラシー教育プログラム」では、かかわることで互いを理解し、医療従事者と障害者双方にとって有意義な医療のあり方を考えた。「保護者と作る子育て支援プログラム」では、保護者同士の語り合いが、見通しを持った子育てや居場所作りにつながり、こども・保護者・学校への理解が深まった。養護教諭や学校精神科医主導の研修会から、先輩の保護者が子育てリーダーとなって後輩の保護者にアドバイスする会へと進化した。
