项目摘要

阪神淡路大震災や新潟県中越地震で被災した患者への調査では、「薬を持ち出せなかった」、「薬の使用の可否がわからなかった」など、被災した患者の薬についての不安や問題点が挙げられ、災害時の医薬品供給のみならず平素からの薬剤の自己管理や災害時にも対応できる服薬指導の必要性が指摘されている。また、今回の東日本大震災では薬事法第49条第1項の規定における「正当な理由」に該当し、処方せんの交付が困難な場合に該当された場合は必要な処方せん医薬品を販売又は授与が可能となったが、その場合も災害時には病院・診療所と保険薬局との連携やお薬手帳はさらに重要となることが考えられる。今回、保険薬局における病院・診療所との情報共有の実際や災害対策の現状について愛媛県内の保険薬局478施設にアンケート調査を実施し、297枚(回収率62%)の回答を得た。患者のお薬手帳の持参割合は20～39%を選択したのが98施設(33%)と最も多く、このままでは病診・薬薬連携による情報共有は不十分であることが示唆された。また、お薬手帳の利便性を理解できている患者は全体の1～19%と少ないと薬剤師は感じており、災害時などの緊急時におけるお薬手帳の有用性について理解できていない患者が多いことが考えられた。さらに主に応需する病院の採用薬の情報は「郵送やFAXなどの通知文書によって把握している」231施設(約78%)が大半を占め、次に「医薬品集」33施設(11%)であった。一方、愛媛県薬剤師会のサイボウズ(グループウェア)を使用できる施設が半数以上あり、迅速な情報はFAXやインターネットによる複数の伝達網構築が有用と思われる。
