目的:微量アルブミン尿(MAU)が慢性腎臓病の早期の診断マーカーになると提唱されているが、尿中漏出アルブミンの変性に伴う抗原抗体反応の低下が、既存の免疫測定のロスを生み、不正確な測定になる可能性があった。そこで本研究は、秋田大学医学部附属病院で腎移植を施行された患者を対象に、尿中MAUの測定法による比較を行った。方法:MAUの測定には免疫学的測定法(サイアスALB-M、関東化学、abMAU)及びゲルろ過クロマトグラフ法(アキュミン^【○!R】、ニプロ、akMAU)を用いた。随時尿のMAU測定であり、同検体の尿中クレアチニン値で補正し、abMAU指数及びakMAU指数として算出した。移植後半年以上経過した腎移植患者48名の随時尿(H23/12/1～22)を本研究に用いた。結果:abMAU指数及びakMAU指数は平均101mg/g(中央値35.6[2.3-803])及び平均112mg/g Cr(中央値43.2[9.7-1060])であり、両者平面プロットには有意な相関がみられた(r^2=0.896)。またabMAU/akMAU比は平均1.7倍、最大5.0倍の差異があり、腎移植患者においても測定方法が結果に大きく影響を与えることが示された。次にMAUが軽度腎機能低下の指標であることから、abMAU指数が正常範囲0-29.9mg/gであって、akMAU指数が30.0mg/g以上の患者を抽出した。その結果48名中6名該当し、軽度腎機能低下患者として把握できた。6名の平均推定CCr値は67.5mL/minであり、その内5名は腎保護作用を持つアンギオテンシン受容体拮抗薬の服用があったが、残り1名には無かった。同薬剤を含む高血圧治療薬を考慮する必要が認められた。本研究からゲルろ過法によるMAU測定の有用性が示されたが、軽度腎機能低下のアセスメントと移植腎予後予測因子の解明に向けた取り組みが必要と思われる。

Objective: Micro-urine (MAU) is an early diagnostic tool for chronic kidney disease (CKD). It is possible to detect MAU in urine with low antigen and antibody reaction, and to detect MAU incorrectly. This study compares the measurement of MAU in urine with that in patients undergoing kidney transplantation at Akita University Hospital. Methods:MAU determination, immunological assay (ALB-M, Kanto Chemical, abMAU) and the "MAU" method (AMAU). R],$>, akMAU) are used. The MAU of urine is measured at any time, and the MAU index of urine in the same sample is corrected. The MAU index and the MAU index of urine in the same sample are calculated. Forty-eight kidney transplant patients (H23/12/1~22) who had passed through the kidney transplantation more than half a year after transplantation were selected for this study. Results: The abMAU index and akMAU index averaged 101mg/g(median 35.6[2.3-803]) and 112mg/g Cr(median 43.2[9.7-1060]). The abMAU/akMAU ratio varied from 1.7 times to 5.0 times on average, and the results were significantly affected by the method used in kidney transplant patients. Patients with MAU, abMAU index and akMAU index above 0- 29.9 mg/g and 30.0 mg/g were selected. Results 6 out of 48 patients with renal dysfunction were diagnosed. The average estimated CCr value of 6 patients was 67.5 mL/min, and 5 patients had renal protective effect. It is necessary to consider the treatment of hypertension with the same medicine. The purpose of this study is to evaluate the usefulness of MAU assay in evaluating renal dysfunction and the significance of the analysis of prognostic factors after renal transplantation.
