项目摘要

【研究目的】保険薬局において、適正な調剤や服薬指導を行うために患者情報を収集することは重要であるが、その確認手段は主に問診とお薬手帳に限られ、確認が十分に行えない場合もあると考えられる。そこで、本研究は現状の紙媒体であるお薬手帳以上の情報量を持たせ、さらに簡単に読み込みが可能となるQRコードを利用したe-お薬手帳の開発を試みることにより、必要な患者情報を保険薬局や他病院等で正確かつ簡便に共有可能とし、医療安全に貢献することを目的とした。【研究方法】システムの構築に当たり、保険薬局における患者情報収集とお薬手帳の電子化に関して事前調査を行った。東京都文京区、台東区薬剤師会所属の保険薬局197施設を対象にアンケート形式として実施、回収した。調査項目は調剤時に必要となりうる患者情報に関しての薬局における有用性、お薬手帳記載の有用性、実際のお薬手帳への記載状況および患者情報把握困難の経験の有無、現状のお薬手帳の問題点、お薬手帳電子化の有用性および問題点などとした。さらに、電子媒体としての携帯端末やタブレット型コンピューターの利便性について検討した。【研究成果】アンケートの回収率は25.9%であった。患者情報の中で、疾病名、肝・腎機能、血圧、血糖値、脂質、尿酸値、アレルギー・副作用歴、他薬局薬歴、および入院中薬歴については85%以上が有用であると回答した。また、これらの情報がお薬手帳に記載されていれば有用との回答も60%以上あった。しかし、現状では、これらの情報がお薬手帳に記載されていないため困った経験も少なくなく、特に疾病名、入院中薬歴はそれぞれ96%、84%の薬局がほとんど記載なしと回答した。お薬手帳が電子化されれば有用であるとの回答は61%と多く、その理由として患者情報の共有化と一元管理、より詳細な情報が入手できる、持参忘れの際でも確認可能、携帯性向上などが挙げられた。以上のことから、お薬手帳を含めた患者情報の電子化は保険薬局に必要な患者情報の共有化や一元管理のために有用になり得ると考えられた。また、電子媒体として、携帯端末やタブレット型コンピューターの利便性が高いと考えられた。
