医薬品の関連する医療過誤は医療機関の安全管理体制に加えて、患者側の要因の影響を受ける。医薬品の関連する医療過誤の中でも、小児に関連する事例は少なくない。しかし、日本において、小児の薬物治療に関連する医療過誤について、その事例を詳細に解析した報告は少ない。本研究は小児に関連する処方および調剤までの医薬品の動線における医療過誤を誘発する要因を解析することを目的とした。2010年8月から2011年7月の期間に、浜松医科大学病院で実施された薬剤師による疑義照会と調剤後の監査時に発見した調剤ミスについて集計を行い、その内訳について小児患者(15歳未満)と成人患者(15歳以上)との間で比較を行った。調査期間における処方に関する疑義照会の総件数は1089件であった。小児患者を対象とした疑義照会は124件あり、そのうち投与量に関するものは50.8%を占めていた。成人患者では投与量に関する疑義照会は全体の24.5%であった。また、調査期間における調剤ミスの総件数は984件であった。小児患者への処方の調剤ミスは60件あり、そのうち計数・計量に関連するものは36.7%を占めていた。成人患者への処方では計数・計量に関するものは54.3%であった。また、小児患者への処方の調剤ミスについては、散剤や水剤に関連したものは、15.8%を占めていた。一方、成人患者への処方では散剤や水剤に関連したものは3.9%であった。以上より、医薬品の関連する疑義照会や調剤後の監査時に発見される間違いは、小児患者と成人患者とでは異なる傾向があった。このことから小児における医薬品の関連する医療過誤の防止には、成人とは異なる対策が必要である可能性が示された。

Medical errors associated with medical products increase the safety management system of medical institutions and affect the patient side. There are very few cases of medical errors caused by medical products. For example, in Japan, medical errors related to small children's drug treatment are analyzed in detail and reported in detail. The purpose of this study is to analyze the main causes of medical errors associated with the development of drugs. From August 2010 to July 2011, Hamamatsu Medical University Hospital conducted a survey of patients with childhood (15 years old) and adults (15 years old and older).
