项目摘要

【研究目的】薬物の母乳への移行性に関して、臨床で有用となる情報が報告されている薬物は限られている。臨床現場では、薬物治療が必要な基礎疾患をもった産後の患者が授乳を希望することも多い。そうした母親からは、薬物の母乳移行性の情報が求められている現状があり、薬剤師として信頼性の高い情報提供の必要性を感じている。そこで、本研究では必要な情報収集の手がかりとして、当院で行っている妊娠・授乳に関するお薬相談の実態を調査し、伝達する情報の構築を開始した。【研究方法】独立行政法人国立成育医療研究センターの事業の一つである厚生労働省事業「妊娠と薬情報センター」でのお薬相談に対し、当院でも拠点病院として事業に加わっていることから、その相談内容についてまず調査を行った。【研究成果】・患者の服用薬剤数は4～6剤が約半数を占め、特に薬剤数が多いと妊娠前から薬剤に関する不安をより感じやすいと考えられた。・薬の種類は、精神科用薬が相談の上位を占めた。多剤併用療法や、疾患の特性により相談件数が多かった可能性が考えられる。また、抗てんかん薬(バルプロ酸など)も上位を示した。抗てんかん薬には催奇形性が問題となるものもあるが、発作を起こさないための薬物治療も重要であり、患者からの相談希望が多かったと考えられる。・授乳に関しては、出産が近づいてからの相談が多かった。胎児への薬剤の影響についての不安が、出産が近づくにつれて、母乳育児が可能かどうかという不安へ移行したものと思われる。授乳に関する相談は、薬剤を使用するには母乳を中止しなければならないと思っている患者が多い印象を受けた。そのため、臨床報告が乏しい薬剤のみではなく、母親が不安を感じている場合には濃度測定を行うなど、対象薬剤を限定せずに母乳への移行性を評価してもよいと思われた。
