【研究目的】薬剤師は患者の薬物療法に対して薬学的観点から様々な介入(薬学的介入)を行い、医薬品の有効性と安全性の確保、医療経済面を含めた医薬品適正使用に努めている。本研究では、薬学的介入を行った結果としての臨床的有用性とその医療経済的効果を客観的に評価し、薬剤師による臨床薬剤業務の意義と必要性に関する科学的根拠を示すことを目的とする。【研究方法】2011年8～10月に京都大学医学部附属病院で集積した入院患者に対する薬学的介入事例を対象とし、医薬品による副作用・相互作用の未然防止、早期発見、重篤化防止につながった事例を抽出した。これら薬学的介入事例の臨床的有用性を個々に評価するとともに、介入件数と薬剤師側の要因(1ヶ月の病棟滞在時間、1ヶ月の薬剤管理指導件数、薬剤師としての経験年数)との相関を解析した。また、病棟に薬剤師が滞在・配置されているメリットについて、医師・看護師に聞き取り調査を行った。【研究成果】調査期間中の薬学的介入件数は1116件、医師の受入率は94.1%であった。このうち、医薬品による副作用・相互作用の未然防止は126件(11.3%)、早期発見は66件(5.9%)、重篤化防止は89件(8.0%)であり、薬剤師が被疑薬の減量・中止や副作用の対症療法を提案し、実施された結果、薬物治療を継続できた例が多くみられた。薬剤師別の薬学的介入件数は、病棟滞在時間、薬剤管理指導件数と正の相関をもつ傾向を認めた。また、医師・看護師からは、病棟に薬剤師が滞在・配置されているメリットとして、薬について速やかに相談できてよい、副作用・相互作用の評価が正しく行える、薬に関わるリスクが低下する等の意見があげられた。薬剤師が病棟に常駐し、患者の病態や治療方針をよく理解した上で、他の医療スタッフと協働して薬物治療の質的向上に努めることが、臨床的に有用であることが示唆された。今後、介入事例の医療経済的効果を検討する予定である。

[Objective] The aim of this study is to improve the effectiveness and safety of medical drugs, and to improve the quality of medical drugs.
