重度聴覚障害児の人工内耳(CI)装用が患児と家族の生活の質にどのように影響を及ぼすか明らかにするために、CI装用本人と保護者による自己評価を行い関連する要因について分析した。対象はCI装用後1年以上が経過した就学後のCI小児49例(平均10.7歳)とその保護者である。QOL評価はPedsQL日本語版を用いた。本検査は身体・感情・社会・学校の4つの領域からなり、計23項目の質問文で構成されている。分析に際し、失語症構文検査における読解力がレベルIII未満、又は絵画語彙検査における語彙年齢が6歳未満の者を除いた。その結果30例が分析対象となり、得られたQOL評価得点と語音聴取能、言語能力や教育環境で比較検討した。CI小児QOL評価得点は平均91.2点(1SD7.9)であり、領域間に差を認めなかった。対して保護者QOL評価得点は平均88.0点(1SD9.8)であり、領域間に有意な差を認めた。多重比較の結果、身体領域に比し社会・学校領域が有意に低下していた(p<.05)。CI装用年齢や聴取能・語彙力・構文力とCI小児・保護者QOL評価得点に相関は認められなかった。就学先で通常学校群と特別支援学校群の2群に分けて下位項目を分析したところ、CI小児では通常学校群と特別支援学校群ともに領域間に差を認めなかったのに対し、保護者では両群ともに領域間に有意な差を認めた。多重比較の結果、通常学校群では学校領域が他の領域に比し有意に低下し(p<.05)、特別支援学校群では感情領域が他の領域に比し有意に低下していた(p<.05)。CI小児QOL得点は一様に良好であった。対して保護者QOL得点は領域毎に差を認め、教育環境により異なる傾向を示した。以上の結果から、CI装用児の就学後支援は本人のみならず、保護者に対するサポートが重要であり、そのアプローチは教育環境により異なる可能性が示された。
