神経疾患あるいは昏睡状態などを原因とする『不随意の咬み締め』によって、自分の歯で歯肉などを傷害して、口腔内裂傷を起こしてしまう患者も多数存在する。これらの口腔内裂傷を予防するためには、「口腔内に装着するプロテクター」の使用が有用である、対象患者の大部分は、不随意の咬み締めと同時に開口制限も有するため、その場合は、プロテクター作成に必須の印象採得が非常に困難である。院内他科との連携も含め、こうした患者の治療を行うに当たって解決しなければならない重要な問題として、「プロテクターに用いる適切な材質と印象採得法の検討」が挙げられる。そこで、「不随意の咬み締めによる咬傷を予防するためのプロテクターの作成法を確立」することを目的として、下記二点について検討した。①咬み締めによる口腔内裂傷を予防するのに必要十分な強度を持つ材料(あるいは材料の組み合わせ)②開口制限患者の印象採得を容易にする方法材料の選択に関しては、強度が高ければ高いほど咬み締め予防には有効であるのは明らかだが、それは同時に装着に伴う違和感を増大させるとともにプロテクターによる口腔軟組織傷害(潰瘍など)を惹起する可能性もある。特に昏睡状態患者では痛みを訴えることが出来ないため、口腔軟組織傷害が重篤化する恐れが高い。そこで、プロテクターは弾性のある合成樹脂材料を用いて作成した。開口制限を有する患者に対する対応としては、これまでに当科で研究・改良されてきた分割トレーによる印象採得法を応用した。本実験においては、シミュレータの開口を制限(1センチを想定)した状態に設定し、開口制限患者の印象採得を模擬的に行い、良好な結果を得た。
