(目的)腎移植前後でのHHVー6,7の動態と臨床症状との関係を明らかにすること。(方法)腎移植前後で経時的に採血し、ウイルス抗体価の測定ならびにウイルスDNAの検出をPCR法を用いて行う。(結果)両側嚢胞腎のため1歳6カ月時からCAPDを施行していた5歳男児にたいして、父親をドナーとする生体腎移植を行った。移植前の抗HHV-6抗体価は陰性で、移植後11日目の血液からHHV-6を分離した。この時期に一致して、移植腎の拒絶反応、38度以上の発熱、間質性肺炎、痙攣、肝機能異常、下痢を認めたため、HHV-6に対して有効であるとされているGanciclovirとγーglobulinの併用療法を施行し、症状は改善した。同時に、HHV-7およびサイトメガロウイルスについても同様の検討を行ったが、いずれもウイルスDNAはPCR法で陰性であった。(考察)これまで我々は、HHV-6の再活性化時に有意に移植腎の拒絶反応を認めることを報告してきたが、今回、移植に際してHHV-6の初感染を認め拒絶反応のみならず重篤な症状をきたした小児例を経験した。移植医療の進歩に伴い、小児の腎移植患者も年々増加傾向にある。その場合、今回経験したような重篤な症状を呈すると考えられるHHVー6に関して、更なる研究を要すると考えられる。今回治療として、抗ウイルス剤とγ-globulinの併用療法を用いたが、さらに症例数を増やして検討する必要がある。また、移植前の抗体価によっては、何らかの予防策を講じることを考慮している。

This time, the transplant recipients refused to respond to the initial infection of HHV-6. This time, they refused to respond to the symptoms of reactivation. The transplant doctors are in the process of increasing the number of patients who have received transplant from year to year. This time, the symptoms of severe illness are related to each other. The symptoms show that the symptoms of HHV are serious. This time, the number of cases of the disease caused by the use of anti-drug therapy, anti-drug therapy, gamma-globulin therapy and the number of cases of the disease was significantly higher than that of the control group. Before transplantation, the antibody was detected, and the preventive measures were taken.
