慢性C型肝炎症例において、血中IL-1ra値がインターフェロン治療効果に及ぼす影響、その産生増加機序について検討し、以下の新たな知見が得られた。尚、検討には診断時および経過観察中に採血が必要と思われた際に得た血清の一部を用いたが、その際に採取した検体の一部を本検討に用いることを患者に説明し承諾を得た。1慢性C型肝炎のインターフェロン治療効果予測因子としてのIL-1ra値の臨床的意義治療開始前の血中IL-1ra値はインターフェロン有効群に比し無効群で有意に高値を示し、多重ロジステイック解析により血中IL-1ra値がインターフェロン治療効果予測因子として有用であること、血中IL-1ra値をHCV-RNA量、ゲノタイプのウイルス因子と組合わせることにより治療効果予測の精度が上がることが明らかとなった。また、低ウイルス量症例では血中IL-1ra値の効果予測因子としての有用性はより高いことが示され、そうした症例ではウイルス側因子のみならず宿主の免疫学的因子もインターフェロン治療効果に関与していることが示された。2血中IL-1raの産生増加機序の解析IL-1β、IL-10、免疫複合体はそれぞれ単球からのIL-1raの産生増強効果を有するが、慢性C型肝炎症例のそれらの値と血中IL-1ra値の相関を検討したところ、いずれも著明な正の相関は認められず、IL-1raの産生増強には他の因子が関与していると考えられた。
