18例の脳外科手術症例に於て40Hz-ASSRの測定を試みたが,安定した波形が得られたのは3例のみで,手術室というノイズの多い場所での臨床応用は容易ではないことが判明した.そこで,脳波,聴性中間潜時反応について検討した.脳波に関しては多くの報告があり,一般に麻酔深度の判定は困難との結論が出されているが,静脈麻酔薬propofolのみで麻酔する場合には麻酔深度の評価が可能と思われた.周波数解析により判定は容易となれば臨床応用可能である.聴性中間潜時反応はASSRと同様聴覚刺激によって誘発されるものであるが,より安定した測定が可能で,通常の全身麻酔では麻酔状態から覚醒する課程で波形の変化が明瞭で,覚醒の予知も可能であった.そこで脳波を持続的に監視しながら聴性中間潜時反応の結果を間欠的に検討することにより麻酔深度を評価しつつ,propofolを主麻酔薬とする全身麻酔を行ったが,麻酔深度を適切にコントロールし,必要に応じ覚醒状態に導けるようである.そこで,propofolのみによる麻酔で自発呼吸下の管理を試みたが,自発呼吸下で満足される麻酔深度を得ることが可能であった.propofolによる麻酔の麻酔深度評価に脳波,聴性中間潜時反応は有用である.また,術中覚醒を要する開頭術ではpropofol単独での麻酔により自発呼吸下で十分な麻酔深度を得ることが可能と考えられる.症例を重ね,さらに検討中である.
