项目摘要

歯周病関連菌に対する歯周病患者の血清抗体価を調べ、その特異的抗体について検討を行った.また,患者血清抗体が病原菌の凝集を阻害するかについても検討した.その結果歯周病患者の血清IgG抗体価は,健常者と比較して有意に高かった.菌種別では,Actinobacillus actinomycetemcomitans(Aa)の血清型aおよびcに高い抗体価を示す患者は,限局型若年性歯周炎の病態を示す場合が多かった.Aaに対する血清IgG subclassと病型とに偏りがみられた.臨床症状では,歯周病患者の血清抗体価と骨吸収速度,骨吸収率との間に相関が認められたが,動揺指数,プローブ後出血指数とは相関はなかった.歯周病関連菌に対する血清IgG抗体価,とりわけAaに対する抗体価は,若年性歯周炎,急速進行性歯周炎などの早期発症型歯周炎の診断に有用であることが示唆された.さらに,Fusobacterium nucleatum(Fn)の特異因子である,L-アルギニン感受性赤血球凝集素に対する歯周病患者の血清IgG抗体価は,成人性歯周炎の患者において高く,Fnの赤血球凝集反応を阻害した.以上のことから,病態によって,歯周病患者の歯周病関連菌に対する血清IgG抗体価は異なり,また,その病態特有の歯周病関連菌,特異因子が存在することが示された.しかし,血清IgG抗体価の偏りが歯周病型に存在する傾向があることから,遺伝性あるいは家族性歯周疾患の発症メカニズムとの関係を検討する必要があると考えられる.

To regulate the serum antibody levels of patients with dental diseases related to periodontal disease-related bacteria and conduct comprehensive research on other specific antibodies. In addition, the patient serum antibody can inhibit the aggregation of pathogenic bacteria. Results: Serum IgG antibody levels in patients with dental disease were significantly higher than those in healthy subjects. The serotype a and c of Actinobacillus actinomycetemcomitans(Aa) are high in antibody levels, and patients with limited type of periodontal disease are often infected. Clinical symptoms: serum antibody, bone resorption rate, bone resorption rate and correlation. Serum IgG antibody levels associated with periodontal disease are useful in the diagnosis of chronic periodontitis, rapid progressive periodontitis, and early onset periodontitis. In addition, the specificity factor of Fusarium nucleatum(Fn) was found to be a factor that inhibited the immune response of Fn to hemagglutinin in serum IgG antibodies in patients with periodontitis and in adults with periodontitis. The above results indicate that there are different serum IgG antibody levels in patients with periodontal disease associated with pathogenic bacteria, and specific factors exist in patients with periodontal disease associated with pathogenic bacteria. There is a tendency for serum IgG antibody levels to be biased towards peridental disease patterns, and it is necessary to investigate the relationship between genetic and familial peridental disease patterns.
