【目的】IDDM患者の歯周病の病態を整理し,歯周病に対する治療および管理を確立することを目的とした。【方法】歯周病に対する認識を高めるためIDDM患者を対象に歯周病についての講演を本年度は下記のごとく行い、歯周病についての臨床検査および各種宿主生体防御機能検査・IDDM患者の歯周病の臨床的病態の整理を行った。岡山小児糖尿病協会主催サマーキャンプ(1996年7月)広島もみじの会集会(1996年9月)近畿つぼみの会集会(1996年9月、12月)【結果】1.IDDM患者の歯科検診および口腔内状態1995年から約170名の歯科検診を行い、ほとんど全ての患者に歯周炎の発症を認めた。各種歯周病関連細菌に対する血清IgG抗体価で個体差は大きいが総じて高かった。2.歯周病についての臨床検査および各種宿主生体防御機能検査本院IDDM患者歯周病外来を受診した22名の患者の内レントゲン診査において早期発症型歯周炎と考えられる4名の患者を発見した。患者7名(早期発症型歯周炎4名、歯肉炎3名)の細菌検査では全員から3種の主要歯周病関連細菌を検出した。リンパ球の機能検査において血清中のTNF-αの濃度が高かった。【考察と結論】IDDM患者は、同年代の健常者に比べて、歯周病関連所見が特徴的であり、歯周病をIDDMの合併症と考えることができる。IDDM患者におけるEOP発症のメカニズムは本合併症を解明する鍵となると考えられる。また、血清中のTNF-αの濃度の上昇によって歯周組織の破壊がもたらされた可能性が示唆された。IDDM患者歯周病外来を受診した患者については、定期的検診・メインテナンスを行っているが、今後も広報活動を続けていく必要がある。

[Objective] To sort out the pathologies of IDDM and to establish the treatment and management of IDDM. [Methods] A high level of awareness of dental disease in IDDM patients was reported. Clinical investigations of dental disease in IDDM patients and various host organism defense functions were carried out. Clinical pathology of dental disease in IDDM patients was analyzed. Okayama Children's Diabetes Association (July, 1996) Hiroshima Children's Association (September, 1996) Kinki Children's Association (September, December, 1996) Results 1. Dental examination of IDDM patients and intraoral status: In 1995, about 170 IDDM patients underwent dental examination and the development of periodontitis was recognized. The serum IgG antibody level of various disease-associated bacteria varies greatly from individual to individual. 2. Clinical investigation of periodontitis and evaluation of host organism defense function in IDDM patients. 22 patients with periodontitis were diagnosed. 4 patients with early onset periodontitis were diagnosed. Bacterial examination of 7 patients (4 patients with early-stage periodontitis and 3 patients with periodontitis) showed that all patients had 3 kinds of bacteria associated with periodontal diseases. The concentration of TNF-α in serum increased significantly in the functional study of the tumor. [Conclusion] IDDM patients have the characteristics of IDDM complications compared with healthy people in the same age. EOP symptoms in IDDM patients are the key to understanding the underlying complications. The increase of TNF-α concentration in serum indicates the possibility of tumor growth in peripheral tissues. IDDM patients are diagnosed during the week, and regular visits are needed for future reporting activities.
