目的 歯周病の病態の解析のモデルとして、遺伝的要素が大きく影響している早期発症型歯周炎患者の家系調査をおこない、各々について一般臨床検査、細菌検査、各種宿主生体防御機能検査の面から調べた。方法:研究計画・方法に記載のとおり披験者は早期発症型歯周炎患者である娘を含む1家族で、母親(RPP)、娘(LJP)、兄(periodontally healthy)、弟(LJP)の4名である。結果 1.細菌検査で、被験者全員の歯周ポケットからActinobacillus actinomycetemcomitansを検出した。母親の深い歯周ポケットからblack-pigmented anaerobic rodsを多く検出した。2.好中球機能検査において、母親、兄、弟の好中球の走化能が低下していた。3.リンパ球機能検査において、母親、娘、弟のT4/T8比が低下していた。4.末梢血IgG抗体価では、母親、娘のPorphynomonas gingivalisの値が高かった。5.被験者のHLAクラスII抗原のDrw52とDQlが共通であった。考察 1.歯周病の初期感染においてActinobacillus actinomycetemcomitansは重要な役を果たしていると考えられる。2.この家族において好中球の走化能の低下は歯周病易発症性のリスク因子の1つである可能性がある。3.T細胞のT4/T8比の低下はもう1つのリスク因子と考えられる。4.HLA-DQlが早期発症型歯周炎の発症に関っている可能性がある。

Objective To analyze the pathogenesis and genetic factors of periodontitis, and to investigate the family history of patients with early onset periodontitis, including general clinical examination, bacterial examination, and various host organism defense functions. Methods: The study plan and method recorded that there were 4 patients with early onset periodontitis, including 1 family member, mother (RPP), mother (LJP), brother (periodically healthy) and younger brother (LJP). Results 1. Bacterial detection and detection of Actinobacillus actinomycetemcomitans in all patients. There are many black-pigmented anaerobic rods in my mother's deep middle dental week. 2. Good ball function test, mother, brother and good ball walking ability is low. 3. The T4/T8 ratio of mother, mother and younger brother was lower than that of the control group. 4. The value of peripheral blood IgG antibody is very high. 5. The HLA II antigen Drw52 and DQl of the affected persons are common. 1. Actinobacillus actinomycetemcomitans are important in the early stage of dental disease. 2. The family has a good chemical energy and a low risk factor for disease. 3. T cell T4/T8 ratio is lower than T4/T8 ratio. 4. HLA-DQ1 is a potential candidate for early onset periodontitis.
