舌は嚥下運動において重要な役目を担う器官であり、舌の悪性腫瘍切除後は嚥下機能が低下する場合が多い。そこで、今回我々は超音波断層撮影により舌切除後の嚥下時の舌運動を観察した。使用機器はアロカ社製SSD-650を用い、探触子に7.5MHz電子コンベックスタイプを使用した。予め健常人において様々な量の食物で舌運動を評価したところ、本装置では2m1の水の嚥下時における舌運動が最も明瞭に観察することができた。従って、舌切除患者に対して、その条件での嚥下時間・舌背部の移動量等の変化を手術直後から経時的に観察し健常人のそれと比較した。対象は、1996年4月〜12月までの間に、舌腫瘍にて舌切除を受けた9人とし、その内2人は部分切除により再建を施行せず、6人は舌を半側以上切除し欠損部に再建を行った。結果として、部分切除を行った症例では患者自身の訴えとして手術部の突っ張り感は残存するものの、嚥下時間・移動量とも比較的早期に回復する症例が多く、手術より6ケ月後の検査ではほぼ正常に近い機能まで回復していた。しかし、舌を半側以上切除し再建を行った症例では、前者に比して明らかに機能の回復が遅く、嚥下時間・移動量は術後半年ほどの間は徐々に回復はするものの、移動量が健常人の半分以下といったように舌運動は著しく悪かった。また、その後も検査し得た2例では舌運動に関して著明な回復は見られなかった。その原因として、これらの症例はただ単に舌を大きく切除しただけではなく、頚部郭清術を伴ったpull through方式による手術法が多かったため、舌筋のみならず口腔底の筋群をも切除したことが一因と考えられる。また、頚部郭清術を行った症例では所属リンパ節に転移を伴っているために、放射線の術後照射の対象となることが多く、放射線治療の瘢痕治癒による咽頭部筋群の運動障害も嚥下運動を低下させている一つの要因と思われた。
