ポリ-L-乳酸(PLLA)は吸収性の高分子精緻材料である。これまで、われわれは骨折および顎変形症の手術においてPLLAプレートおよびスクリューを使用し、良好な結果を得ている。さらに、PLLAを延伸形成することによって作製した膜を骨誘導再生法に応用することを考察し、すでにPLLA膜がGBR法に用いる隔壁としての条件(生体親和性・封鎖性・スペース確保)を満たしていることを確認した。今回の研究においては、スペースを確保して骨組織の再生を持つだけではなく、ここにリコンビナントヒト骨形成タンパク(rhBMP)を応用し、積極的に骨の誘導をはかる試みを行った。<方法>実験にはビ-グル犬を用い、両側下顎第三および第四前臼歯を抜歯した後、抜歯窩が上皮にて被覆されるまで約3ヶ月間放置した。この部位の辺縁切除を加え、ここに厚さ100μmのPLLA膜を適合させた。さらに骨欠損部充填材としてrhBMPを用いた場合の骨組織の再生状態を確認した。術後時間経過とともに新生される骨の状態は、軟X線写真・pQCT・三次元CT・硬組織研磨標本により評価した。<結果>rhBMP非充填群、充填群ともに経時的に骨組織の形成が認められたが、rhBMP充填群では非充填群と比較して著明な骨形成が観察された。また、rhBMP充填群における骨形成量は濃度依存的であった。以上により延伸成形PLLA膜は顎骨欠損に対するGBR法に有用であり、rhBMPを応用することにより、より優れた結果が得られることが判明した。今後これらの結果を口腔外科領域における機能的顎骨再建に応用し、臨床での実用化の可能性を追求したい。

In the following time, we have passed the X-ray photo, pQCT, 3D CT hard tissue grinding standard, which is the standard for new bone grinding. & lt; results: the bone tissue of gt;rhBMP non-filling group and filling group formed bone formation test, and the bone formation test of rhBMP filling group showed bone formation test. The bone formation of the rhBMP filling group is dependent on the bone formation. The results of the above PLLA film extension test show that the GBR method is useful, the rhBMP method is useful, and the results show that the results are accurate. In the future, the results show that the possibility of bone reconstruction and bed reconstruction in the field of oral surgery is highly desirable.
