本研究の目的は、高いレベルでの談話産生能力に認知能力はどのように関わるかを脳損傷患者において検討することにある。10名の軽度失語症患者を対象に、4種類の談話課題、すなわち一枚の絵について叙述する物語課題、特定の日常的行動を行う手順を話す手続き課題、ビデオドラマを見て内容を叙述する物語課題、生活のアイディアを提供するビデオ番組を見て内容を説明する手続き課題を施行した。さらに、全対象者にWilsonら(1996)の開発した、遂行機能をみる検査バッテリー、The Behavioural Assessmet of the Dysexecutive Syndrome(BADS)、およびレーヴン色彩マトリックス検査を施行した。談話は、話を構成する各部分の出現の有無とその順序性の点から検討を行った。この結果から以下のことが示唆された。1. 失語症重症度では同程度の軽度失語症患者でも、BADSの結果では差が見られた。2. BADSで障害が明らかであった患者には、談話の手続き課題において、手順の説明の省略がみられた。しかし、物語課題、特にテレビドラマの叙述においてはストーリーの流れに大きな問題はみられなかった。3. BADSにおいて障害が示唆されても、談話課題においては適切に話すことが可能であった患者もみられた。軽度の失語症患者の場合、BADS検査は言語機能の影響を大きく受けることなく、行動の調整、計画といった高次の認知機能を検討できる検査であること、また、そうした機能が、複数の段階を追って説明を組み立てて行く必要のある手続き談話の産生と関わりがある可能性が示唆された。しかし、今回は少数例の検討であったため、現在、追加的にデータ収集を進めている。また、非失語の遂行機能障害患者の談話も検討する予定である。
