弾性裏装材で裏装した義歯の床下組織における顎機能時の負担圧様相について検討し,中心咬合位および右側方咬合位での義歯に加わる局所機能圧の緩衝効果を解明する目的で,無歯顎患者に近似した形態,顎粘膜厚径および被圧変位性を有する上顎負担圧測定シミュレーションモデルを用いて,デュロメータA硬さの異なる2種類の弾性裏装材を裏装した義歯に対応する顎粘膜面および顎骨面における局所機能圧時の負担圧様相について検討し,以下の結論を得た.1.顎粘膜面および顎骨面の負担圧は,中心咬合位および側方咬合位ともに弾性裏装しない義歯に比較して有意に小となった.2.弾性裏装した義歯での顎堤頂部の負担圧は,顎粘膜面および顎骨面のいずれにおいても,中心咬合位および側方咬合位ともに他の計測部位と比較して大となった.床縁部,口蓋部および頬側部の負担圧は,顎堤頂部に比較して小さく,ポストダム部の負担圧は最も小となった.3.弾性裏装した義歯での顎骨面の負担圧は,中心咬合位および側方咬合位ともに顎粘膜面のそれに比較して有意に小となった.4.デュロメータA硬さの小さい弾性裏装した義歯での顎粘膜面および顎骨面の負担圧は,デュロメータA硬さの大きい弾性裏装した義歯に比較し,有意に小となった.以上から,弾性裏装材は,局所機能圧時の上顎総義歯における床下組織への負担圧を軽減する緩衝効果を認め,支持機能に適した負担圧配分を行うことが明かとなった.その効果は,デュロメータA硬さの小さい弾性裏装材の使用によって,より高くなることが判明した.さらに,弾性裏装した上顎総義歯では,裏装しないレジン床義歯に比較して,より厚い緩衝腔の設定の必要性,ならびにレジン床義歯と同程度のポストダム設定の必要性が示唆された.
