本研究は、全身麻酔による手術患者が術後に排痰を行う際に、心身の苦痛が少ない咳を促すために作成した看護プログラムが実践的に臨床応用できることを明らかにするために、プログラムを臨床的に適用したものと適用しなかったものとの間で比較検討することを目的として行った。看護プログラムは、(1)咳に関するオリエンテーション、(2)催咳法(咳の前の深吸気、腹筋を使った咳、創部の支持)、(3)咳の前後のリラクセーションを取り入れた。対象は、開腹手術を受ける20歳以上65歳未満の成人で、術後に自力で排痰可能と考えられる人のうち、研究参加の承諾を得られた人とした。プログラムを適用する群を実験群、適用しない群を対照群とした。調査は、咳の仕方として胸郭・腹部の動きを、また咳の発生状態として吸気・呼気流量、指尖容積脈波、腹筋の表面筋電位を測定した。咳に伴う身体的・心理的苦痛として、痛み、疲労感、咳に関連した不安等について面接を行った。測定時期は、麻酔終了後24時間、48時間、72時間、96時間とした。分析は、二元配置分散分析をを用いた。その結果、実験群が対照群に比べて有意に高値を示したものは、吸気流量(p<0.001)、腹直筋の筋電位(p<0.02)、腹部の動き(p<0.002)だった。呼気流量、胸郭の動きは実験群が対照群に比べて高い値を示したが、有意差はなかった。また、咳の前後で痛み・疲労感が増強した割合は実験群が対照群に比べて少なかったが、有意差はなかった。咳に伴う心理的苦痛では、咳をすることに対して否定的な感情を抱く割合が実験群に比べて対照群に有意に(p<0.01)多かった。以上から、プログラムの深吸気の練習は吸気筋の機能を促進し、術後の咳の場面においても吸気効率を高めたこと、および催咳法は腹筋を効率よく用いて咳を行うことに貢献したこと、創部の支持・咳の前後のリラクセーションは咳を行う際の痛みの増強を抑制したことが考えられ、プログラムの初期の目的は達成されたと考えられた。
