喀痰からの結核菌耐性遺伝子の検出に関する検討

対象:当院肺結核患者で、Rifampicin(以下RFP)耐性の感受性検査結果を示し、過去にPyrazinamide(以下PZA)の使用歴を有する症例のうち、喀痰からの持続排菌陽性例6例を対象に行った。方法:患者からの喀痰を採取し、NALC-NaOH処理を行った後、一部は小川培地にて培養検査を行った。培地にて増菌した結核菌を用いRFP感受性検査を、PZAに対してはPZA液体培地にて感受性検査を施行した。培地にて増菌した結核菌の1コロニーと、患者喀痰をNALC-NaOH処理を行った検体は直接アンプリコアマイコバクテリウム用検体前処理キット(日本ロシュ)にて結核菌群のDNAを抽出した。DNA抽出後、あらかじめ作成していたrpoB遺伝子、ならびにpncA遺伝子に対するprimerにてPCRを施行した。PCR産物に対してSuprec02カラム(宝酒造)を用い精製後、PCR-direct sequence反応を行い、塩基配列の検討を行った。結果:培地からのRFP感受性検査の結果は全て完全耐性を示していた。PZA感受性検査の結果は全て感受性を認めた。培地と喀痰からのrpoB遺伝子の変異を確認したところ、6例中4例が同一の遺伝子変異を示しており、変異部位はcodon526、codon516、codon513であった。考察:喀痰、培地ともにRFP耐性を示すものの、6例中2例は異なる変異を示していた。これは採取される喀痰と培地との菌株の違いが原因と考えられ、必ずしも喀痰から培地と同一の菌株を採取されるとは限らないため、今後喀痰より耐性検査を施行する際には注意が必要と考えられた。

Subjects: Results of susceptibility test for resistance to Rifampicin(hereinafter RFP) in patients with tuberculosis in our hospital were shown. In the past, Pyrazinamide(hereinafter PZA) was used in all cases. Methods: After the patients were treated with NALC-NaOH, a part of Ogawa culture was examined. RFP susceptibility test was conducted for the growth of tuberculosis bacteria, and PZA susceptibility test was conducted for PZA liquid culture. The DNA of tuberculosis bacteria was extracted by NALC-NaOH treatment in culture and sputum. After DNA extraction, PCR was performed on the primer. PCR products were purified by PCR, PCR-direct sequence analysis was carried out, and gene alignment was discussed. Results: The results of RFP sensitivity test showed that the resistance to RFP was complete. The results of PZA susceptibility test showed that PZA susceptibility was completely recognized. The difference between the two genes was confirmed in 4 of 6 cases, and the difference between the two genes was confirmed in codon526, codon516 and codon513. The results showed that there was no obvious difference in the tolerance of RFP in two of the six cases. The reason for the violation of the resistance test of the strain should be examined. The resistance test of the strain should be carried out in the future.
