项目摘要

本研究の目的は、1)集中治療部における医師の処方行動を、専門集団の合意形成という視点から検討し、形成された合意がどの程度適切と考えられるか、さらに不適切な合意を避けるために必要な条件・情報などを明らかにすること、2)必要と思われた情報などを提供することで実際に合意形成がどう変化するか検討することの2つである。平成12年度は、これらの合意形成に向けて、ICU内での情報提供がどのように判断に活かされるかを検討した。この調査に基づいて、実際に臨床で必要とされる情報を検索しまとめ上げるために、どの程度の時間を要するかについて検討したが、予想外に時間を要し、得られた情報も十分判断に活かされる情報ではないことが多いという結果になった。このような中で、処方行動の変容について調査を行った。特に、術後感染予防のための抗生物質の投与に関して調査を行い、院内ガイドライン作成や院内感染症対策チームに対する推奨によって、抗生物質投与のタイミングの変化が変化しつつあることが確かめられた。今回の研究の問題点は、1)介入の前後での比較試験であり、時期の変化による変化と介入による効果が厳密には分けられないこと、2)事後調査ではICU内での合意形成に関する細かな評価が難しく、十分な検討が加えられなかったこと、などが挙げられる。しかし、医療チームの処方行動を変える上で、特にチーム内で評価が大きく分かれる点に関しては、医療情報を扱いやすいように整備し提供することが、重要であることが確かめられた。
