项目摘要

【本年度の研究の内容】1998年度中に以下の二つの研究を行った。1)1996年の名古屋市での院外心停止傷病者に関して1年後まで予後を検討した。2)1998年1ヶ月にわたって院外心停止患者の救急隊が現地で記録した心電図を集め、その初期調律を検討した。【研究結果】1)1996年に搬送した心肺停止傷病者1190例のうち、1年後生存が確認されたのは44例(3.7%)であった。院外心停止の予後調査の国際規格であるUtstein templateに準じて集計を行うと、心原性で目撃者ありと判断された319例のうちでは4例(1.3%)、社会復帰と判断されたのは3例(0.9%)であった。2)1998年4月の1ヶ月の間に記録された心電図74件のうち、除細動の対象になる心室細動または心室頻拍はわずか5件(7%)であった。【考察と今後の展開】1997年度の研究結果でも示されたが、院外心停止患者の予後は過去の海外の報告に比して決して優れてはいなかった。米国でもニューヨークなどの都市部では地方都市に比べて悪いことが知られている。また、今回の心電図の検討でも示されたように、最も蘇生の対象になると考えられている心室細動などの除細動の適応となる患者が名古屋市では少ないことも蘇生率を引き下げる原因と考えられた。今後はこの結果をふまえ、心室細動の時点で救急隊が傷病者にアクセスできるように、救急司令センターからの蘇生法の指導などの発見者の蘇生施行率向上への取り組み、救急隊の特定行為の指示の簡略化・迅速化、さらに救急隊の特定行為の拡充などを考慮する必要性があるものと思われた。この調査結果は、厚生省の研究班報告、名古屋市救急指導会議の報告などに利用され、上記の対策も名古屋市では具体化されつつある。さらに、名古屋市救急対策室の協力を得て今後も継続的な調査・評価体制を維持することが確認されている。

In the future, the results of this study, the timing of ventricular movement, the emergency team's response to the patient, the guidance of the emergency commander's health law, the implementation rate of the discovery of the emergency team, the simplification and quickness of the instructions for the specific behavior of the emergency team, and the necessity of considering the specific behavior of the emergency team. The results of this investigation include the report of the research team of the Ministry of Health and Welfare, the report of the Nagoya City Emergency Guidance Conference, and the report of the Nagoya City Emergency Guidance Conference. The Nagoya City Emergency Response Office has cooperated to maintain the investigation and evaluation system for future projects.
