1.目的:積雪地域の高齢者への年間を通じた地域参加支援のあり方を明らかにすることをめざし、地域参加型機能訓練事業(以下、事業)の効果評価に関する視点の明確化と、効果的な展開方法に関する指針のための基礎資料を得ることを目的とした。2.方法:1積雪地域において、平成11年度より住民参加型の事業を月1回実施した。内容は運動、交流、創作活動などである。データは、事業運営、参加者、地域スタッフ(ボランティア)に関するフィールドノート記載及び諸記録、ビデオ・写真撮影、録音により収集した。参加者の健康指標(健康感、生活・知的機能、外出頻度等)、スタッフの意識は構成的質問により収集した。分析は、初回から20ヶ月後に変化がみられた事項と内容、事業の効果と思われる事象を明らかにした。3.結果:1)参加者の状況(1)参加登録者は初回の11人から20ヶ月後には30人となった。(2)参加契機は当初は保健婦からの紹介が多く、次第に参加者や地域スタッフからの紹介が増加した。(3)参加者は毎月、登録者の60〜70%であった。不参加理由は非積雪期は所用、積雪期は体調不良が多かった。(4)送迎が必要なものは初回(非積雪期)は1名だったが、20ヶ月後(積雪期)には15人となった。(5)参加継続者には生活・知的機能、外出頻度が維持、向上したものが多かった。(6)参加者間の交流が増加した。2)地域スタッフの状況(1)登録者は初回の15人から倍増した。(2)地域スタッフの毎月の満足感、有用感は高かった。3)地域への波及(1)住民の交流や広報により事業が周知されるようになった。(2)本事業をモデルに、他地域での同様の事業開催が計画された。4.考察:事業の効果評価において、参加者の健康の向上、スタッフのエンパワメント、地域への波及という視点は効果の一端を示すと考えられた。事業の効果的な展開には、1)企画・運営における参加者、地域スタッフの協働、2)小地域を単位とすること、3)季節、環境をふまえた送迎システムが重要である。
