本研究の目的は、欧米諸国とりわけ英国の緩和ケア医療施設の発展経緯と施設計画の現況について明らかにすることである。本年度は、主として以下の3つ項目に関する結果が得られた。1)英国の緩和ケア医療施設の発展の経緯昨年度得られた資料と本年度新たに入手した資料を総合し、英国における緩和ケア医療施設について、1970年代以降の設置形態、施設数の変遷等に関するデータの分析を行った。施設数の増加をみると、1980年に59施設(1373床)であったものが、1990年には146施設(2590床)、2000年には233施設(3364床)に達している。1980年からの20年間で約4倍の増加をみせていることがわかった。2)近年の施設計画St.Chiristopher'sHospice(1967)を設計し、その後も長く緩和ケア医療施設の計画・設計に携わってきたCIiveHicks氏が設計した最新の施設「Hospicein the Weald(1998)」を訪問し、同氏の計画した施設の実際の使われ方を調査した。同氏が提案する各部門の明確なゾーニング構成が的確に機能していること、地域コミュニティの拠点として施設が認識され、活用されている実態が把握できた。3)小児専門ホスピス英国のホスピスの発展経緯をみると、1983年から小児を対象としたホスピスの建設が見られる。1990年代後半から急激な施設数の増加が見られ、2000年には21施設(159床)にまで達している。そこで、小児専門ホスピスとして特徴を把握するため、Little Heven Children's Hospice(Essex)を調査対象として、施設計画の経緯、施設の利用状況に関するヒアリング調査を行った。その結果、1)対象疾患は悪性新生物よりも神経難病が多い、2)神経難病の治療を目的とした診療室やプールを設置する、3)兄弟で同じ病気に罹患した患者の利用が多いため、隣接する個室の壁に扉を設けて相互に行き来ができるような構造にしている、4)家族のレスパイトケアとしての役割を重視し病室と同数の家族室を設置する、等の小児ホスピスの特徴があることがわかった。

The purpose of this study is to clarify the current status of palliative care development and design in the United Kingdom and Europe. The results of the following three projects have been achieved during the year. 1) The development of palliative care facilities in the United Kingdom was analyzed in relation to changes in the form and number of facilities in the 1970s. The number of facilities increased from 59 in 1980 (1373 beds) to 146 in 1990 (2590 beds) to 233 in 2000 (3364 beds). In 1980, it increased by about 4 times in 20 years. 2)The design of St. Chiristopher's Hospital (1967) is the latest in the design of the Hospital in the Weald(1998). In the same way, each department has a clear understanding of the structure of the project, and the actual situation of the project can be grasped. 3)In 1983, the United Kingdom began to build a small number of projects. The number of emergency facilities increased in the second half of the 1990s, reaching 21 facilities (159 beds) in 2000.
