平成12年度の研究では,抗リン脂質抗体症候群(APS)の主要な抗体の1つであるループスアンチコアグラント(LA)のうち,抗プロトロンビン抗体(anti-PT)には結合プロトロンビンの種特異性,抗体とプロトロンビンの結合様式の相違より4種類のanti-PTが存在し,個々のanti-PTが血管内皮・単球・血小板の抗血栓性・易血栓性に及ぼす影響につき検討した.LAにはanti-PTの他,もう一つの主要抗体として抗β2-glycoproteinI抗体(aβ_2GPl)が存在する.そこで平成13年度ではこのaβ_2GPlの血管内皮。単球・血小板に及ぼす影響につき検討し臨床症状との関連を比較した.(1)本人の了承を得て採血したLA陽性血漿よりProtein Gカラムおよびヒトβ2-GPl affintyカラムを用いてaβ_2GPlを精製した.(3)その結果,aβ_2GPlはHUVECのProtein C活性化作用を有意に抑制したが(P<0.01),血管内皮のTF産生作用・plasminogen activator inhibitor(PAl-1)産生作用には有意の影響は与えなかった.また単球のTF活性にも有意の影響は与えなかった.一方,aβ2GPlは血小板活性化を有意に亢進した(Pく0.05).(4)これまでの検討で,LAのうちaβ_2GPlを有する症例では動・静脈血栓症発症危険度が有意に高値であったもののヒトプロトロンビン結合型anti-PT(type-1)と比較すると危険率は低地であった.平成13年度の研究より,aβ2GPlの血栓形成機序は血管内皮のProtein C活性化抑制効果および血小板活性化促進作用によるものと考えられたがその効果はanti-PT(type1)より軽度であった.今後,LAをsubtypeに分類することにより個々の症例の血栓症発症の予知が可能となる可能性が示された.
