Nitrofen 100mg投与によって誘導されたラット横隔膜ヘルニア(CDH)の低形成肺における肺動脈壁肥厚の病因を解明する目的で、細胞外基質蛋白質のturnoverを決定するmatrix metalloproteinases (MMPs)とその特異的抑制剤であるtissue inhibitor of metalloproteinases (TIMPs)の関わりを免疫組織化学的に検討した。妊娠20日目に帝王切開にて胎仔を回収し、左CDHラットの左肺を摘出し、直ちにOCT compondに包埋後、ドライアイス・エヌヘキサン中で急速凍結させた。この凍結組織をクレオスタットにて冠状面で厚さ5μmに薄切し、アセトン固定後、MMP-1(collagenase1)およびTIMP-1の一次抗体を用い酵素抗体法を行った。いずれも室温における湿潤な環境下で1〜12時間反応させた。二次抗体としてアミノ酸ポリマー化酵素標識抗体を用い、DAB-H_2O_2溶液で可視化した。水洗後hematoxylinで後染色し脱水、封入、透徹した。なお、陰性コントロールとしてマウスIgGを一次抗体の代わりに用いた。その結果、CDH群(n=5)および非CDH群(n=5)において、内弾性板外側の肥厚した中膜に存在する平滑筋細胞間にMMP-1陽性と思われる類円形ないし短紡錘型細胞が散見されたが、両者で明らかな差は観察されなかった。一方、TIMP-1はCDH群(n=3)および非CDH群(n=3)においては中膜、外膜には明らかな陽性細胞は観察されなかった。今回の免疫組織学的検討からはCDHにおける肺動脈壁肥厚の病態を示唆する所見が得られなかった。今後、例数の増加と共に、技術面の改善ならびに他の細胞外基質代謝酵素による検討も必要と思われた。

Objective: To elucidate the etiology of pulmonary arterial wall hypertrophy induced by Nitrofen 100mg, matrix metalloproteinases (MMPs) and tissue inhibitors of metalloproteinases (TIMPs) were determined by immunohistochemistry. On the 20th day of pregnancy, the fetus was cut open and the left lung was extracted. After the OCT compond was embedded, the fetus was frozen rapidly. The frozen tissue was cut into 5μm thick coronal section. After fixation, the primary antibodies to MMP-1(collagenase1) and TIMP-1 were detected by enzyme antibody method. In the room temperature and humid environment, it takes 1 ~ 12 hours to return to work. Secondary antibodies are visualized in DAB-H_2O_2 solution. After washing, hematoxylin is dyed, dehydrated, sealed, and thoroughly. The first generation of IgG is used in the production of negative antibodies. The results showed that CDH group (n =5) and non-CDH group (n=5) were positive for MMP-1 in smooth muscle cells, and there were scattered and abnormal cells in smooth muscle cells. One side, TIMP-1 CDH group (n = 3) and non-CDH group (n=3) were detected in the middle membrane and outer membrane. Immunohistological examination of CDH showed that the pulmonary hypertrophy and pathological changes were observed. In the future, the number of cases will increase, the technical aspects will be improved, and other extracellular matrix metabolic enzymes will be discussed.
