III.研究結果(回収率37.7%、有効回答率92.0%)(1)糖尿病患者への医療状況・対象323所における1ヶ月あたりの初診患者の総計数は1400名、再診者は22484名であった。・26%の施設が検診・受診を中断する人が多いとし、理由は患者の「自己管理不足」が72%と最も多く、その他「就業関係問題・自覚症状改善」などがあった。対応策では148所が「放置」状況であった。・糖尿病医療の内容では「網膜症管理・糖尿病教室・運動指導・シックデイ対策・フットケア・こころのケア」については約60%〜80%が実施できていない状況であった。(2)糖尿病医療における地域連携・紹介先・紹介を受ける先で最も多かったのは「病院」であり、「有床・無床診療所」は各々30%前後であった。「保健所・行政」は少なかった。・紹介先の選択基準は「専門医がいる」が76%と最も多く、「近隣の施設・総合病院・患者の希望・合併症治療や検査・糖尿病教室がある」があった。・紹介状の活用現状については「ほとんど持参」が医師、看護師、栄養士3者とも30%未満と少なかった。・医師・看護師の紹介状にかった内容は「紹介目的・診断名・検査結果・現病歴・薬物療法・血糖値・HbA_1c・合併症の有無」が70%〜80%と多く、栄養士では「食事療法」が77%と最も多くその他の項目は医師や看護師と比較して少なかった。一方、紹介を受ける場合では3者とも全体的に実際に欲しい情報量が増えていた。(3)地域連携に関する関心・地域連携のための連絡調整室や部門があるのは23所(8%)で、その是非について3者とも約80%以上が「必要」としていた。また、看護師と栄養士は紹介状による継続や連携について90%以上が必要性を感じていた。・地域連携を行うグループ構成は、3者とも「各施設・機関の糖尿病医療に関わる様々な職種で構成されたチーム」で連携を図る場合が70%以上と最も多く、この中には「同職種間」の専門性の中で連携を図ることも必要と考えている場合もあった。IV.考察、連携システム構築は今後の課題
