平成18年度の質問紙調査結果から、スタンダードプリコーション(以下SP)遵守に影響を与えている管理的要素として、管理者のSPに関する理解不足、感染対策マニュアルの内容の不備・活用の問題、物品の供給不足等が明らかとなった。今年度は、管理者の知識・意識に介入することによって改善が期待できる点を明らかにするとともに、介入しても感染管理の改善が困難である揚合の要因について分析した。管理者のSPについての知識・意識に介入するための方法として、ワークショップ(以下WS)を2回開催した。1回目のWSの内容は、X県内の訪問看護ステーション管理者を無作為に選出し同意が得られた8名で、2回のWSに参加するよう依頼し、参加前後で質問紙による追跡調査を実施した。質問紙による回答は、WS開催前に質問紙A(SPを遵守する上で問題となることとその原因)、2回目WS終了10日後に質問紙B(WS参加後、立案した感染予防策)、2回目WS終了3ケ月後に質問紙C(実行している感染予防策、継続できなかった感染予防策とその原因)を無記名で郵送するよう依頼した。その結果、WS終了2ケ月後までに改善できたことは、感染予防のマニュアル作成、手指衛生の物品・個人防護具(以下PPE)・針刺し防止物品等の購入、毎日のガウン洗濯等で、改善が見られない内容にスタッフの手指衛生・PPE不適切があり、その要因はWSの伝達だけではスタッフの認識が低いというものだった。
