最終年度の研究目的は2つある。まず1つは、前年度からの課題でもあるポケット部を清浄に保つためのカテーテルを用いたポケット内の創洗浄によってサイトカイン値に良い影響を及ぼすかを検証することである。そして、2つめは、洗浄の有無による治癒過程を比較し、さらにアクション・ベースト・コスティングを算出し、評価することである。方法は、まずポケットを有する褥瘡を対象にポケット内の創洗浄を行うことによって、4週間ごとにサイトカインの経時的変化を調査した。そして、洗浄の効果を評価するために、ポケットを有する褥瘡を対象を封筒法によって従来の洗浄方法群とポケット部を清浄に保つための洗浄方法群の2群に分け、1ヶ月間週に1回写真撮影をした。2群間における治癒過程に影響を及ぼす要因に差がないことを確認してから、ポケット面積の変化を比較し治癒過程を評価した。さらに、人件費と部材費をこの間調査し、アクション・ベースト・コスティングの手法を用い経済評価を行なった。なお、対象者からは、研究協力の承諾を得た。サイトカインの変化は、5部位のポケットを有する褥瘡で調査した。洗浄を開始すると1週間後より創傷治癒を促進させる成長因子のb-FGFとPDGFが増加した。褥瘡部ではポケット覆蓋部の肉芽組織が増殖しポケット覆蓋部と創底の肉芽組織との接着を認めた。次に洗浄の有無による治癒過程と経済評価の比較は、各群5名5部位の褥瘡を調査した。2群間における、年齢、ブレーデンスケールの得点、栄養状態、褥瘡の深達度と部位には差がなかった。ポケット面積は、有意な差はないが洗浄群の縮小面積が大であった。経済効果は、費用効果では洗浄群が高く、増分費用効果比は洗浄群が洗浄しない群に比べて効果大・費用大の象限に該当し効果ありと評価できた。以上より、ポケットを有する褥瘡は、ポケット内部もカテーテルを用いて洗浄することがサイトカインという分子生物学的視点から有効といえた。また、今回対象数は少ないが、この洗浄ケア手技は、ポ治癒過程を促進させ、かつ経済効果も高いことより、有効なケア技術の開発につながったといえる。
