本研究の目的は、ストーマ周囲皮膚を観察することにより皮膚障害を判断し、皮膚障害がある場合にはケア計画の立案もできるシステムを開発することである。前年度までは、ストーマ周囲皮膚障害の状態ならびに発生部位を言語化し、皮膚生理機能と併せて皮膚障害の病態との関連性を見出した。しかし、病態は3種とストーマ周囲皮膚に一般的に認めるといわれる皮膚障害の病態を網羅していなかったため、前年度の調査を継続した。その結果、対象者は74名から111名となり、病態はカンジダ、粘膜移植、壊疽性膿皮症等の8つを追加することができた。病態増加に伴い、水疱、膿疱、潰瘍が皮膚障害の所見が追加された。次に、ストーマ周囲の皮膚障害を観察するスケールの作成のために、ストーマケアのエキスパートである医師・看護師にインタビューを行った。その結果、ケアに連動させるためにはストーマ周囲皮膚をストーマ近接部、皮膚保護剤部、皮膚保護剤外部の3つに区分し、皮膚の状態を観察する必要性が見出された。さらに、皮膚障害の状態をスケール項目とした時に治癒期間を指標にすると、利用者がケア介入の緊急度等を理解しやすくなると考え、状態別に統計学的に重みづけを行った。その結果、紅斑、水疱・膿疱、びらん、潰瘍・不良肉芽・粘膜皮膚移植・肥厚において、それぞれ2、3、4、40と重みづけがされた。ただし、3つの皮膚区分における治癒期間の重みに差はなかった。最後にケアに連動するシステムとするため、調査した3つのストーマ周囲皮膚の区分と紅斑、水疱、膿疱、びらん、潰瘍、不良肉芽、粘膜皮膚移植、肥厚の組み合わせから、病態との関係性を導き出した。さらに、提供されたケア、ならびにエキスパートの見解をふまえ、アルゴリズムによってケアを導き出すツールを開発した。
