近年、がん患者に対する心理的介入が行われ、一部の研究では、生存率、心理指標、生理指標の改善効果が示されている。しかし、効果的な心理的介入方法は現在まで確立されていない。本研究では、早期乳がん患者を対象としたwriting法による自己表出(これまでの辛い出来事等を自由に記載する方法)に焦点を当てた介入の心理指標、生理指標、脳機能に対する効果を検討し、writing法のRCTへの実行可能性を検討した。対象は以下の基準を満たす者とした:(1)研究登録時点で40歳から70歳以下の女性、(2)乳がん根治術を受け、補助化学療法終了後6ヶ月以上経過している者、(3)エストロゲン・プロゲステロンの各レセプターが陰性の者、(4)糖尿病・精神疾患・脳血管疾患の既往歴が無い者。本研究は対照群を設定せず、対象者全員に連続する3日間×各20分間のwriting法による介入を行った。介入効果を検証するために、介入開始前(心理指標・生理指標、脳機能)、介入3ヶ月後(心理指標のみ)、介入6ヶ月後(心理指標、生理指標、脳機能)に評価を行った。統計解析は、心理指標に関しては一元配置分散分析を行い、生理指標に関しては、Paired t-testを行った。本研究対象者は6名であり、全ての介入及び調査を完遂した。心理指標に関して、分散分析の結果、抑うつ(p=0.61)、不安(p=0.43)、QOL(p=0.76)は有意な交互作用を示さなかった。生理指標に関して、Paired t-testの結果、ACTH(p=0.077)、NK細胞活性(p=0.32)、コルチゾール(p=0.26)は有意な差を示さなかった。脳機能は画像解析のため、現在定量的な集積を行っている。本研究により、早期乳がん患者に対するwriting法の心理・生理指標の改善は示されず、writing法のRCTへの実行可能性は低いと考えられる。

In recent years, there has been a lot of psychological intervention, a study, survival rate, psychological index, and physiological index. The method of psychological intervention is now available to make sure that it is necessary to do so. In this study, patients with breast cancer and early breast cancer showed that the focus of writing method was psychological index, physiological index, machine performance index, writing method and RCT method. For example: (1) to study women under 40 years old and below 70 years old at the time of entry, (2) to receive radical mastectomy, and to assist in the treatment of breast cancer more than 6 months later, (3) to have sex disorder in each group of patients, and (4) those with diabetic mental disorders and vascular diseases who have not been affected in the past. In this study, according to the photo group setting, the whole staff of the camera will be involved in the bank by using the writing method for a period of 20 minutes each within 3 days. The results of intervention were as follows: before the beginning of intervention (psychological index, physiological index, machine ability), 3 months after intervention (psychological index, physiological index, machine ability), and 6 months after intervention (psychological index, physiological index, machine ability). Statistical analysis, psychological index analysis, unitary allocation dispersion analysis, physiological index analysis, and Paired t-test analysis. In this study, 6 elephants were involved in the study, including the intervention and the completion of the intervention. Psychological indicators such as the results of dispersion analysis, inhibition of anxiety, anxiety, and intentional interaction of QOL indicated that there was no significant difference between the two groups. The physiological index, Paired t-test results, ACTH (pP0.077), NK cell activity (pP0.32), and purposeful difference (pP0.26) showed that there was no significant difference between the two groups. The machine can be used to analyze images, and now there is a quantitative collection of images. In this study, patients with early breast cancer were treated with writing method, psychophysiological index was improved, and writing method was RCT with low possibility of operation.
