社会の高齢化に伴って認知症患者は増加し続け、2015年には262万人に達すると推定されている。その予防対策は急務であるが、その為には認知症の危険因子を明らかにしていく必要がある。これまでの既存研究では、認知力低下者には、基本的な日常生活動作や、手段的な日常生活動作が低下している者が多いことが知られているが、より詳細な生活習慣と認知症低下との関連を明らかにした研究は少ない。そこで、我々は平成元年から脳卒中・心筋梗塞の発症登録を実施している地域をフィールドに地域住民における認知症低下とその危険因子に関する調査を実施した。対象者は65歳以上の地域住民を無作為に抽出し、インフォームドコンセントをとり、同意を得られた者とした。抽出は年齢階級別に層化して行った。認知力低下者(Mini Mental State Examinationが21ポイント未満)の割合は85歳未満で男女とも5%であったが、85歳以上では男性28%に対して女性は38%であった。これは男女の平均寿命の違いによると考えられるが、一方で女性の認知障害の低下が目立ち、女性に対するケアの問題が浮き彫りになった。性、年齢階級、最終学歴別にみた認知力低下の割合は、男女とも高校以上の学歴を持っている群と比して、中学生(旧制も含む)までの群の方が認知力低下者の割合は高かった。認知力維持者と比較して、認知力低下者には基本的日常生活動作や手段的日常生活動作が低下している人が多かった。加えて、多くの趣味を持っている者ほど認知力機能の低下者が少なかった。これらの分析より、高齢者の認知機能低下には、日頃の生活習慣が関連していることが明らかになった。分析結果は、地域住民の方や調査に協力いただいた方に対して報告会を実施し、研究成果を還元した。また、本研究は滋賀医科大学倫理委員会で承認を得ている。
