【目的】わが国を含む東アジア地域では、一般集団を対象とした矯正治療必要性についての疫学調査は少なく、国際的に用いられている矯正治療必要度(Index of Orthodontic Treatment Need : IOTN)を用いた報告はほとんどなされてない。今回我々は、はじめてIOTN原法に準じて、直接法によりわが国の小学6年生と中学1年生を対象とした多地域調査を行った。また、学童と保護者に対して、学童の、歯並びと咬合状態に対する満足度や矯正治療必要性ならびに咬合異常に対する価値観等についての質問票による調査を行った【資料】新潟県農村地域のA,B,C小学校193名、新潟市中心部のD小学校141名、首都圏近郊の埼玉県E小学校140名、東京都心のF小学校131名、福岡県郡部にあるG中学校1年生82名のうち、承諾の得られた487名を対象とした。【方法】IOTNは、機能と形態面からの矯正治療必要度とされるDental Health Component(DHC)と、審美面のAesthetic Component(AC)から構成されている。これらについて診査を行った。また、学童、保護者に対する質問表による調査を行った。【結果と考察】矯正治療経験者は14.5%であったが、地域によって7.0〜25.6%と異なっていた。矯正治療未経験者について検討したところ、推奨されているDHCによる判定で「矯正治療必要性あり」は34.1%であった。しかし、ボーダーラインとされるDHC3は、41.4%と高い数値を示した。また、DHCとACを合わせた判定では、35.5%が「矯正治療必要性あり」とされた。DHCの咬合異常(不正咬合)の内容は、叢生17.4%、過蓋咬合13.9%、萌出余地不足13.2%、過度のover jet 10.1%、交叉咬合2.6%であった。また、質問票から、41.7%の学童が歯並びや咬合状態に不満であり、47.1%が歯科矯正治療を希望していた。【結論】原法に準じて、IOTNの多地域疫学調査を実施し、同時に学童、保護者の意見を調査した。これらは、わが国における歯科矯正治療のニーズとデマンドを考える上で、諸外国のデータとも比較可能な客観的および主観的矯正治療必要性のベースラインデータとなるものである。

[Results] 14.5% of the patients treated with corrective drugs were different from those treated by geographical drugs (7.0 ~ 25.6%). 34.1% of patients who did not receive corrective treatment were judged as "necessary for corrective treatment." DHC3, 41.4%, high value. DHC and AC were judged to be "necessary for corrective treatment" by 35.5%. DHC occlusal abnormalities (malocclusion) were observed in 17.4% of cases, 13.9% of cases, 13.2% of cases with insufficient eruption margin, 10.1% of cases with excessive overjet, and 2.6% of cases with crossbite. 41.7% of the children had no dental problems and 47.1% had no dental problems. [Conclusion] The original method is accurate, IOTN's multi-regional epidemic investigation is implemented, and the opinions of students and protectors are investigated simultaneously. For example, if the patient has been diagnosed as having a dental problem, the patient may be diagnosed as having a dental problem, and the patient may be diagnosed as having a dental problem.
