高齢者の誤嚥は極めて重要な問題であり、市中および院内肺炎の原因の相当数を占めていると考えられる(日本呼吸器学会:市中肺炎ガイドライン2007年、院内肺炎ガイドライン2002年)。誤嚥の存在に対する診断法には、飲水試験、嚥下誘発試験、ビデオフルオログラフィー、Radioisotope (RI)を用いた睡眠中の誤嚥検査があるが、今回我々は、経口(経食道)的に摂取された1,3-β-D-グルカンは血中にはほとんど移行しないが、経気道的に摂取された場合には移行する可能性をある症例から予測し、この現象を応用した誤嚥評価法の開発を試みた。すなわち、誤嚥後に1,3-β-D-グルカンの血中濃度が正常値(20pg/ml以下)を大きく超えて高値を示した症例を経験したため、その原因の検索のため誤嚥した経口栄養剤そのものの1,3-β-D-グルカンを計測したところ、28,600pb/mLと極めて高い値を示し、大豆が主成分である経口栄養剤の誤嚥がその原因と考えられた。一方、経胃的に経口栄養剤を投与している患者5名の血中の1,3-β-D-グルカンを測定しても12.3pg/ml、18.1pg/ml、6.7pg/ml、21.2pg/ml、18.4pg/mlとほぼ正常範囲であった。これらのデータを受け、昨年度はマウスを用いた実験において、やはりβ-D-グルカンが経口的には吸収されず、経気道的にのみ吸収されることを確認した。この実験では、6週齢雄のddyマウスに1,3-β-D-グルカンであるCM-Curdlanを経口投与および経気道投与を行い翌日血中のβ-D-グルカン濃度を測定したところ、経口投与した群では、21.1±28.9pg/mlを示したが、経気道投与を行った群では13,200±15,300pg/mlと極めて高い値を示したのである。よって、今年度は何人かの被検者に試み、臨床応用することを目標とした。まず、β-D-グルカンを多量に含むシゾフィラム(スエヒロダケより抽出)を、誤嚥が予想される高齢者に投与するプロコールを作成し、本学倫理委員会に提出した。シゾフィラムは既に別の薬理作用を目的として、患者に投与されており、経口投与の安全性が証明されているとして、倫理委員会にてこのプロトコールが承認された。その後、数人の誤嚥性肺炎が疑われる患者に投与し、血中β-D-グルカンの値を測定した。現在それらのデータを解析中である。また、上記は画期的方法と考えられ、「多糖を用いた誤嚥検査」として特許出願中である。
