项目摘要

今年度は、昨年度までに確立したヘモグロビン(F-Hb)および乳酸脱水素酵素(LDH)を歯周疾患指標値とした唾液検査値と全身状態、とくに脳血管障害の危険因子となるメタボリックシンドローム(MS)の基準項目との関連を検討した。2007年に一般健診と歯周病唾液検査を同時に受けた成人364名(男性176名:51.7±14.1歳、女性188名:49.3±13.2歳)を本研究の対象とした。唾液は専用ガムにより咀嚼刺激唾液(全唾液)として採取した。歯周病マーカーであるF-Hbはラテックス比濁法で、またLDHはJCCLS標準化対応法によりそれぞれ測定した。その結果、唾液中F-Hbは、肥満あり、ならびにMSありの集団で有意に高い値を示し、唾液中LDHは高脂血症を除くすべての項目で、その有所見者に有意に高い値を認めた。一方、唾液中LDHはMS関連因子(肥満、糖尿病、高脂血症、高血圧症)の重積とともに上昇する傾向にあり(p=0.0020、ANOVA)、MS関連因子の該当数が3以上の集団(359±343U/1)では、該当数が0の集団(235±163U/1)よりも有意に高い状況にあった(p<0.05)。また、歯周病唾液検査の判定(A:異常なし、B:要観察、C:要医療)とMS関連因子(肥満、糖尿病、高脂血症、高血圧症)の該当数の間には統計学的に有意な関連があり(p=0.034、χ^2検定)、歯周病ハイリスクでは、MS基準項目該当数がより多く重積していることが確認された。以上より、歯周病ハイリスク者はメタボリックシンドロームのリスクが高いことが強く示唆された。全身状態の予防医学的観点からも成人の歯周病検診は重要であり、とくにMSハイリスク者では、歯周疾患の予防・改善を目的とした専門的な介入が必要であると考えられた。
