本研究の目的は、高度認知障害を有する高齢患者に対して行ったケアの効果を測定するための適切な次元と方法を見出し、この検討過程から高齢患者のケアの現場で実用可能な測定用具開発のための理論的基盤を明らかにすることである。本年度は、昨年度得られた効果測定の主次元として対象患者の「快適さ=心地よさ」をとりあげ、ケア提供者がチェックするツールに構成して(1)研究者自身が認知症治療病棟で3例の患者に使用、(2)認知症療養病棟看護師により計15例の患者に使用した。これらの結果をもとに専門家会議で妥当性、運用可能性を検討した。ツールを使用した認知症療養病棟看護師へのインタビューでは、簡単にチェックできること、チェックすることで観察視点が明確になったこと、ケア方法を工夫したことなどが語られ、運用可能性は高いことが示された。また、患者の経過からチェック項目の妥当性が考えられた。検討結果をふまえ修正したツールは、対象患者の心地よさを示す表情等6項目と心地よくない状態を示す項目6項目、および日常生活の次元として食事、排泄、睡眠の状態を示す3項目のチェックと、その他気がついた点の自由記述から成る。評価方法は、心地がよい/よくない状態を示す12項目は、そのレベルを「全くみられない」「1度みられた」「複数回みられた」という頻度で示すこととした。また、その性質を把握することでケアの効果の変化をより詳細に示すことができるため、特に印象に残った状態について記述する欄を設けた。このツールが効果測定としての信頼性を得るには測定方法に基準を設けるなどの課題がある。しかし、ケア提供者たちがケアの効果として患者の心地よさをとらえようと努力し、その経過を見ることで、より心地よい状態を目指す取組みを成し得ることが示され、これを理論的基盤として構造化した。
