本研究では、病院と施設の中間的な位置にある介護老人保健施設の特質をとらえ、その機能を活かした独自の看取りケアを創出することを目的として開始した。令和4年度は、介護老人保健施設における看取りケアの現状と課題を明らかにすることを目的に、全国調査を行った。2022年5月～7月に無記名自記式の質問紙調査を行った。全国老人保健施設協会に登録している3600施設より、2000施設を抽出し、郵送法にて施設の看護師に回答を依頼した。分析は、年間の看取り件数が０件（A群）、1～10件（B群）、11件以上（C群）に分けて比較した。その結果、329施設から返送（返送率16.5％）があった。年間看取り件数が記載された回答257について分析した。A群が40（15.6％）、B群が109（40.4％）、C群が108（42.0％）だった。この3群に共通する課題は、「職員教育が不十分」だった。A群のみ「体制が整っていないこと」を多くあげていた。工夫していることやうまくいっていることとして、B群で多くあげられたのは「家族との信頼関係」だった。C群では「職員教育」「職員間の連携」「ケアの質」が多かった。施設での看取りケア推進の課題は、看取りケアの体制を整えることと職員教育や連携の強化であると考えられた。また、自由記述より、回答者は看取りに関する自己研鑽を行っているほか、職場での研修会開催、相談できる専門職の存在などで看取りケアの質向上を図っていた。今後、介護老人保健施設の看護管理者や各職種が認識している課題や取り組みに関する文献から介入方策を検討する。また、介護老人保健施設に配置されているリハビリテーション職の看取りにおける役割や実践内容について、インタビュー調査を行う予定である。

This study is the beginning of the study on the characteristics of elderly care facilities in the middle of hospital facilities. In 2004, a nationwide survey was conducted on health care facilities for the elderly. From May to July 2022, the anonymous self-recording questionnaire survey was conducted. The National Association of Geriatric Care Providers registered 3600 providers, 2000 providers, and caregivers responded to the mail.
