現在、宮城県では年間約3,000人の脳卒中回復期リハビリテーション(以下リハ)を必要とする患者が発生していると試算されている。脳卒中医療は急性期医療、回復期医療、維持期医療に分かれて運営されているが、それぞれの医療は地域の中の異なる施設において行われている。このため、各施設間の連携を効率よく運営していくことが、今後の脳卒中医療において重要と考えられる。本研究では、宮城県において構築された「スマイルネットみやぎ」による脳卒中医療の効率化を検討することを目的とした。「スマイルネットみやぎ」はインターネットホームページ上で脳卒中患者のデータを共有することで急性期医療、回復期医療、維持期医療の各施設を有機的に結びつけることを目的としており、データベースとしての機能と患者紹介システムの機能を有しており、日本インターネット医療協議会の審査で個人情報保護の安全性について適正との判断を受け、平成19年1月より本格運用を開始した。しかしながら、医療情報をインターネット上で使用することは現段階では非常に困難であることが本研究調査で判明した。一例としては、宮城県内の脳卒中急性期病院の核である仙台市立病院では仙台市議会の反対により患者データを電子媒体で院外に持ち出すことができず、「スマイルネットみやぎ」に参加できない状況であることが判明した。これを踏まえて、当研究では電子媒体を用いない脳卒中地域連携の方策を思考し、宮城県脳卒中地域連携クリニカルパス研究会において作成された脳卒中地域連携クリニカルパスの運用状況を調査した結果、宮城県の行政的バックアップも受けて30以上の医療機関で効率的に運用されていることが確認された。以上より現時点ではIT化などの電子媒体を用いない地域連携が最良の脳卒中地域連携診療になることと考えられる。
