前年度に引き続き、研究チームとX県Z市の保健福祉専門職による「合同研究会」を組織し、そこでの定期的な会合での検討を中心に、研究を実施した。その経過と結果は以下のとおりである。1.自治体・介護保険施設等の先進事例の訪問調査と資料収集を行い、プログラム開発の経緯と内容、その実施結果を分析した。その結果、当初考えていた集中的なケアマネジメント体制の確立とともに、住民の生活圏における居住系サービスの整備、ホームシェアリング方式等の入所サービスの柔軟な利用の仕組みの導入、通所療養介護の活用、人材の養成等の重要性が明らかになった。2.「合同研究会」において、継続的に実践事例の検討を行った。その検討から、在宅生活が困難になりつつある要介護高齢者への対応の課題として、看護と生活支援の一体的な提供、在宅医療のネットワークの開発、入院機会の保証などによる安心感の醸成、状態に応じた入所の見直しの必要、季節的な問題への対応、虐待ケースへの対応等の課題が明らかなり、それらの課題の解決に向けての取り組みの方向性についての認識を共有することができた。この研究成果は、直ちに新たなプログラム開発と実践モデルの形成にまでは至らなかったが、今後の研究の展開にむけての多くの示唆を得ることができた。3.施設入所のためのケアマネジメントのプログラムの開発については、特別養護老人ホーム等の専門職員との協議のもとで、ケアマネジメント機関等の相談機関で活用でき、ウェッブ上での公開も可能な形で各施設の概要と特徴等をわかりやすく提示する施設利用支援ツールを開発した。以上の成果をもとに、プロジェクトの総括と結果の公表のためのシンポジウムを現地で開催し、研究成果の地域社会への還元をはかり、また、参加者との討議を通じて、今後の研究の展開の方向について有益な示唆を得る機会とすることができた。
