项目摘要

脂肪塞栓は、骨折等等に伴い生じる可能性のある重要な合併症の一つであり、急死の原因ともなるため、法医学領域で扱われることも少なくない。しかし、現在までそのメカニズムには不明な点が多く残されており、剖検診断も難しい。今回、10週齢前後のWistar系ラットをネンブタール腹腔内投与による麻酔後、腹腔を開放し、下大静脈からオリーブオイルと生理食塩水の10%エマルジョンを2〜10ml注入。注入直後〜6時間後に安楽死させた後、両肺、腎を摘出し、定法によるホルマリン固定の後、オスミウム酸を用いて再固定する方法により脂肪染色し、光学顕微鏡にて検鏡。強拡大1視野中の脂肪滴を数える方法で脂肪塞栓の程度を半定量化することにより、以下の点について明らかにした。1.一般に脂肪塞栓の程度の評価は難しいが、今回用いた方法によりある程度の定量化が可能となる。2.しかし、肺における脂肪塞栓の程度に、それを死因と判断できる程度の閾値のようなものは存在か否かについては、本研究では解明できなかった。ラットの循環血液量を考慮すれば著しく多量の脂質・生食のエマルジョンを注入することによりようやく数匹のラットが死亡したが、脂肪塞栓が死因となるか否かには、単なる物理的な血管の閉塞度以外の因子も寄与しているのかもしれない。3.遠隔臓器の脂肪塞栓について、今回対象としたのは腎のみであったが、10ml注入群で6時間後に認められた以外は、ほとんどの例で認められなかった。きわめて多量の脂質を注入し、数時間経過して、ようやく少量の脂肪滴が肺から他の臓器へ移動したのみということは、たとえば心肺蘇生による肋骨骨折のみでは、脂肪塞栓が生じたとしても肺に限局する可能性が高いことを示唆している。逆に、腎にわずかでも脂肪塞栓が認められれば、塞栓発生後ある程度以上の時間経過が生じていることを示す、いわば生活反応であると考えられる。

The degree of fat plug in lung, the cause of death, the threshold value of the degree of fat plug, the existence of fat plug, and the analysis of this study. The amount of circulating blood is considered to be a factor other than the amount of lipids, raw food, and injection. 3. The fat plug of the remote device is closed, and the current image is closed. The kidney is closed, and the 10ml injection group is closed after 6 hours. A large amount of fat is injected into the lungs, and a small amount of fat is injected into the lungs. The time period after the occurrence of fat plug is longer than the time period after the occurrence of fat plug is longer than the time period after the occurrence of fat plug.
