養護学校において医療的ケアを必要とする子どもに関わる看護師の機能と専門性を明確にする目的で、全国の肢体不自由養護学校への、看護師。養護教諭・教員を対象とした質問し調査を実施し、看護師は99名から回答が得られた。また、質問紙によって得られない現場の詳細や看護師の認識を把握するために、研究の賛同が得られた看護師8名にグループインタビューを行った。質問紙の結果、看護師においては6割が非常勤・パートタイムである状況の中で、教諭や養護教諭とは8割以上がコミュニケーションが十分。まあまあとれていると回答して連携については比較的高く、ケアに看護師としてのアセスメントが生かされているかについては約8割がまあまあできているとしている。そのような中で医師からの指示書のある児童生徒の緊急事態に遭遇したと答えた看護師が約半数おり、主治医からの情報収集や相談は、学校勤務経験の少ない看護師ほど十分でない状況もあった。学校内は、病院内とは異なり、医療者同土の連携はほとんどなく、情報収集源も限られているため、学校内部でのミーティングなども重要である。看護師は7害り程度がミーティングに参加しているのみで、その内容として、行事の参加への対応は約半数、児童生徒の教育・保健計画は1割以下であった。雇用形態との関係で、連携や看護としての専門性の発揮に限界がある実態が浮かび上がっている。グループインタビューでは、学校によって雇用や体制などかなり異なっていたが、どの看護師も、専門性を探りつつ校内の養護教諭や教員との連携を重視して医療的ケアを実施していた。しかし、緊急事態の遭遇もある中、看護師として保険に加入している者は詰り弱であり、今後体制整備とともに、看護師への研修等が課題である。
