特別支援学校における小児看護実習展開の可能性に関する検討

项目摘要

近年子どもも在宅療養化が進み医療的ケアを必要とする子どもも特別支援学校等に通学することが増えている。今回、小児看護実習における特別支援学校での看護基礎教育の実習展開の可能性を明確化することを目的に、全国の特別支援学校の常勤看護師のいる学校の看護師を対象に質問紙調査を行った。その結果として64名から回答を得た(回収率55.7%)。特別支援学校の看護師は、81.3%が小児看護実習の受け入れを依頼された経験はなかった。看護学生に学んでほしいと思う項目は(複数回答)、教諭との連携81.3%、養護教諭との連携78.1%、児童生徒の一般状態の観察76.4%、児童生徒とのコミュニケーション75.0%、児童生徒の家族への対応70.3%、児童生徒の学校での教育・生活67.2%、健康管理体制64.1%であり、看護学生が実習中に体験できる項目で多かったのは、児童生徒とのコミュニケーション73.4%、児童生徒の一般状態の観察54.7%であった。そのほか、見学ならできる項目も挙がっていた。実習指導に関する自由記載では、特別支援学校における実習の意味としてa.看護の視点の広がり、b.障害のある子どもの生活への関心、c.特別支援学校における障害のある子どもへのケア方法、d.特別支援学校における看護職としての機能が挙がり、困難な点としては、実習で目標を達成させられるかや、実習受け入れ体制・状況が整わないことや、子ども達への影響が挙がった。考察として、特別支援学校における実習を受け入れる場合、看護師の雇用状況など受け入れ学校の体制を整えることが課題としてあった。学生に学んでほしい内容は、特別支援学校の児童生徒の理解や他職種との連携など、看護師の役割について学生が学ぶ意味があることを学校看護師も認識していた。また、多くの学校看護師が医療的ケアや児童生徒の教育や生活状況について学生に学んでほしいと考えていた。今後、特別支援学校における実習は、前述の意義等を考慮して目的を特化して行うことで、小児看護の一部として有意義な実習になると考えられた。
